Clinical Characteristics and Bacteriological Profile of Moraxella Keratitis

Abstract: In Japan, Moraxella keratitis occurs in patients with contact lens wear, trauma, and diabetes mellitus. It presents as a small, round, ring-shaped, irregularly shaped, or amoebic-shaped focus. Moraxella species exhibit good susceptibility to fluoroquinolones and aminoglycosides. Because the treatment response may be very slow, these agents should be continued for a long period of time.

“…Moraxella species on the other hand, are a rarer causative pathogen for keratitis with a reported rate of 3.0-3.9% of all cultured proven bacterial keratitis. 3,17 Inoue et al 18 reviewed 30 cases of Moraxella keratitis retrospectively and found that contact lens wear was one of the predisposing factors for this infection but other ocular comorbidities including the use of steroid eye drops and poor ocular surface were also significant contributing factors. When patient age was more than 60 years, systemic comorbidities, such as diabetes, were an important predisposing factor.…”

“…As first described by Morax [1], the genus, Moraxella , appears to have a specific tropism as an ocular pathogen for keratitis [2,3,4,5], conjunctivitis [6], and endophthalmitis [7]. Moraxella keratitis appears to be on the rise based on recent reports [2,3,4,5,8]. Bacterial conjunctivitis is generally self-limiting, but Moraxella conjunctivitis can persist for weeks and presents as a follicular conjunctivitis which can be misdiagnosed as inclusion conjunctivitis due to Chlamydia [6,9,10,11].…”

“…Tan et al [1] recently reported a 12-year analysis of microbial keratitis in a tertiary eye centre (Manchester) in which they found a significant decreasing trend in Gram-positive keratitis along with an increasing trend in Moraxella keratitis. According to the literature and our experience, Moraxella keratitis is known for its chronicity and slow response to treatment [2,3]. In view of their findings, we performed a 10year retrospective analysis (January 2008-December 2017) on the microbiological profiles of microbial keratitis in Sunderland Eye Infirmary (one of the three main eye centres in the North East of England) to determine if the changing trend of microbial keratitis was similar across the UK.…”
