(1) Background: Dysphagia is a clinical hallmark and part of the current American College of Rheumatology/European League Against Rheumatism (ACR/EULAR) diagnostic criteria for idiopathic inflammatory myopathy (IIM). However, the data on dysphagia in IIM are heterogenous and partly conflicting. The aim of this study was to conduct a systematic review on epidemiology, pathophysiology, outcome and therapy and a meta-analysis on the prevalence of dysphagia in IIM. (2) Methods: Medline was systematically searched for all relevant articles. A random effect model was chosen to estimate the pooled prevalence of dysphagia in the overall cohort of patients with IIM and in different subgroups. (3) Results: 234 studies were included in the review and 116 (10,382 subjects) in the meta-analysis. Dysphagia can occur as initial or sole symptom. The overall pooled prevalence estimate in IIM was 36% and with 56% particularly high in inclusion body myositis. The prevalence estimate was significantly higher in patients with cancer-associated myositis and with NXP2 autoantibodies. Dysphagia is caused by inflammatory involvement of the swallowing muscles, which can lead to reduced pharyngeal contractility, cricopharyngeal dysfunction, reduced laryngeal elevation and hypomotility of the esophagus. Swallowing disorders not only impair the quality of life but can lead to serious complications such as aspiration pneumonia, thus increasing mortality. Beneficial treatment approaches reported include immunomodulatory therapy, the treatment of associated malignant diseases or interventional procedures targeting the cricopharyngeal muscle such as myotomy, dilatation or botulinum toxin injections. (4) Conclusion: Dysphagia should be included as a therapeutic target, especially in the outlined high-risk groups.

Amaç: İdiyopatik enflamatuvar miyopatiler (İİM), immün aracılı kas hasarlanması ile giden, proksimal iskelet kaslarının enflamasyonuna bağlı bağ dokusu hastalığıdır. İİM'li hastalarda malignite sıklığı yanında mortalite de genel populasyona göre artmıştır. Çalışmamızın amacı İİM tanısı ile izlenen hastalarda mortalite için prediktif faktörleri tanımlamak ve eşlik eden malignitelerin sıklığı ve tiplerini değerlendirmekti. Yöntem: Ocak 1998 ile Ocak 2020 yılları arasında, üniversite hastanesi romatoloji bölümünde 1975 Bohan-Peter tanı kriterlerine göre İİM tanısı konulup takip ve tedavisi yapılan 53 hasta retrospektif olarak değerlendirildi. Hastaların klinik ve laboratuvar bulguları, malignite varlığı ve ölümü öngördüren prognostik faktörler analiz edildi. Bulgular: Çalışmaya dahil edilen 53 hastanın (32 kadın, 21 erkek) yaş ortalaması 55,24±16,12 yıl, miyopati tanı yaşı ortalaması 44,45±17,62 yıl idi. Miyozit alt gruplarının dağılımı incelendiğinde 27 hasta (%50,9) dermatomiyozit ve 23 hasta (%43,4) polimiyozit idi. En sık görülen klinik bulgu %77,2 ile halsizlik idi. Hastalarda steroid kullanım oranı %98,1 ve azatioprin kullanım oranı %34 idi. Malignite sıklığı %13 (n=7) iken (2 nazofarenks kanseri, 2 kolon kanseri, 1 küçük hücre dışı akciğer kanseri, 1 meme kanseri ve 1 pankreas kanseri) malignite gelişen hastaların yaşı gelişmeyenlere göre daha yüksek ve yine miyozit tanı yaşı da daha yüksekti (sırasıyla p=0,03 ve p=0,02). İİM'li hastalarda bazı değişkenlere göre mortaliteye bakıldığında cinsiyet, hastalık süresi ve miyozit öncesinde malignite varlığı istatistiksel olarak anlamlı farklılığın görüldüğü değişkenlerdi. En sık mortalite nedeni malignite idi. Kohortumuzda 5 yıllık kümülatif sağkalım oranı %80,75 ve 10 yıllık sağkalım oranı %72,68 idi. Sonuç: Sonuç olarak İİM'li hastalarımızda mortalite ve malignite sıklığı genel olarak literatüre benzer şekilde idi. Beklenen yaşam süresi ve mortalite üzerine etkili en önemli faktörler ileri tanı yaşı ve malignite varlığı idi.

Increased Cumulative Incidence of Dermatomyositis in Ulcerative Colitis: a Nationwide Cohort Study

On a molecular level, two autoimmune diseases: ulcerative colitis (UC) and dermatomyositis share common genetic determinants. On a clinical level, case reports evidenced the co-occurrence of these two diseases. We therefore hypothesize that UC is potentially associated with increased cumulative incidence of dermatomyositis. The goals of this retrospective cohort study were to evaluate whether UC is associated with increased cumulative incidence of dermatomyositis independent of sex and age. For comparison, we also assessed the cumulative incidence of polymyositis in UC and control subjects. The study enrolled 3,133 UC subjects and 14,726 control subjects. The cumulative incidence of dermatomyositis was significantly higher in UC than that of control subjects (p = 0.026), but the cumulative incidence of polymyositis was comparable between UC and control subjects (p = 0.596). UC was independently associated with the increased incident dermatomyositis (hazard ratio: 6.19, 95% confidence interval = 1.77–21.59, p = 0.004) after adjusting for sex, age, and concomitant rheumatoid arthritis, systemic lupus erythematosus, and systemic sclerosis. Similar trends of increased dermatomyositis in UC were observed when patients were stratified based on sex and age. In conclusion, our findings suggest that UC is probably associated with increased cumulative incidence of dermatomyositis, independent of sex, age, and concomitant autoimmune diseases.
