A 55-year-old woman presented with progressive enlarging and painful non-healing ulcers on her bilateral lower extremities; biopsy was consistent with pyoderma gangrenosum. Workup for an underlying illness revealed a cavitary lung nodule and an ulcerating mass in the anal canal. Patient did not have any respiratory or gastrointestinal symptoms. Differential diagnosis included inflammatory bowel disease, rectal carcinoma or infection such as tuberculosis, fungal process. Histopathology did not reveal any malignancy, inflammatory bowel disease or infection. Serological studies were positive for perinuclear antineutrophil antibodies specific to proteinase-3 antigen, and the patient was ultimately diagnosed with granulomatosis with polyangiitis. Intravenous pulse dose steroids were initiated followed by monthly pulse cyclophosphamide for 6 months, resulting in rapid and significant improvement of the wounds.

ÖSSZEFOGLALÁSAz ANCA-asszocált vasculitisekben (AAV) gyakran jelentkeznek klinikailag nem specifikus bőrtünetek, melyek a betegség aktivitását jelezhetik. Legnagyobb számban az eosinophil granulomatosis polyangiitissel (EGPA) betegségben látunk változatos léziókat. A bőrtünetek klinikai szignifikanciájáról és kliniko-pathológiai korrelációjáról kevés adat áll rendelkezésünkre. Három betegünk kapcsán bemutatjuk a bőrgyógyászati eltérések mellett a kísérő egyéb szervi manifesztációkat, illetve tapasztalatainkat a kezelés hatékonyságát illetően. Tapasztalataink alapján úgy gondoljuk, hogy a bőrtüneteket az EGPA-ban korai aktivitási markerként értékelhetjük, de az eltérő klinikai megjelenési formák nem befolyásolják a betegség prognózisát és nem mutatnak korrelációt a betegség súlyosságának mértékével. Kulcsszavak: eosinophil granulomatosis polyangiitissel -ANCA vasculitis -bőrtünetek SUMMARYIn ANCA-associated vasculitis cutaneous manifestations are frequently observed. Although, they show no specific features, they can be associated with the activity of the disease. Various skin lesions are more frequent in eosinophilic granulomatosis with polyangiitis (EGPA) than in granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA). Data on the clinical significance of cutaneous manifestations and on the clinico-pathological correlations are lacking. In this article we review the cases of three patients diagnosed with EGPA, presenting their clinical manifestations and the efficacy of the therapy. Based on our experience we conclude that cutaneous manifestations can be considered as an early activation marker for EGPA, but the various clinical patterns do not correlate with the prognosis and severity of the disease.Az eosinophil granulomatosis polyangiitissel (EGPA)korábban Churg-Strauss szindróma, az ANCA asszociált vasculitisek csoportjába tartozik. Ide soroljuk még a granulomatosis polyangiitisszel (GPA, korábban Wegener granulomatosis) betegséget és a mikroszkópos polyangiitist is. Mindhárom betegségre jellemző az ANCA poztivitás mellett a kis és közepes erek nekrotizáló granulomaképződéssel járó vasculitise. Az EGPA diagnózisának felállítása során a 2012-es Chapel Hill-i Konszenzus Konferencia alapján leírt klasszifikációs kritériumokat használjuk. Klinikailag jellemző a beteg anamnézisében szereplő asthma bron-chiale, felsőlégúti érintettség, azaz eosinophil gazdag és nekrotizáló granulomatosus gyulladás a légutakban, a perifériás vérben 10%-ot meghaladó eosinophilia és a glomerulonephritis fennállásakor kimutatható ANCA asszociáció. Az American College of Rheumatology ajánlása alapján a fent említett kritériumok mellett kiemelik a neuropathiát (mono-vagy polyneuropathia), illetve a migráló tüdőinfiltrátum jelenlétét.
