BackgroundMalaria worldwide annual reported cases range between 250 and 500 million and nearly half a million deaths are reported every year. Colombia has a vast expanse of territory with environmental and social conditions conducive to malaria transmission, which is the reason why it has second place in Latin America for the number of cases of malaria.MethodsThis is a retrospective, paired, case–control study that compares patients with severe malaria and malaria patients without mention of complication. Medical records of patients with confirmed malaria diagnosis were reviewed. The objective of this study was to identify factors associated with complicated malaria in three municipalities of Colombia during the period 2009–2013.ResultsA total sample of 180 medical records was collected, 60 corresponding to cases and 120 to controls following a 1:2 ratio. From the total sample, 88.33 % (159) of subjects were originally from Tumaco, 7.78 %, most of the patients of the study (104) were diagnosed and treated in 2013. Among the laboratory findings, the platelet count was found to have statistically significant differences between cases and controls; this was also true for total bilirubin value and transaminases. The most prevalent severity finding was clinical jaundice, followed by elevated bilirubin (25 %) and elevated transaminases (44.4 %). Thrombocytopenia was found in 11/44 cases (20 %) and only five of 60 patients (8.3 %) presented severe anaemia. The multivariate analysis showed that a platelet count <100,000 and clinical jaundice not associated with organ failure, were the two variables associated with severe malaria in the patients described.ConclusionsMost of the cases studied, despite meeting criteria of severity, were shown not to be as morbid or severe as in other countries. Clinical jaundice and thrombocytopaenia are associated with severe malaria, and they can be used by general practitioners or primary care physicians to promptly identify patients who may have severe malaria.

ABSTRACT… Cerebral malaria is a disease entity which we commonly come across and that it should be suspected in every patient with impairment of conscious level and high spiking fever especially with no history of trauma. Early treatment is crucial and can be lifesaving. There are two treatment option in which one is conventional Quinine Dihydrochloride and other artmisinin infusion. Objectives: To study the outcomes of Quinine dihydrochloride and artemisinin practice in patients with cerebral malaria in terms of acceptance and response. Methodology: A total of 78 patients fulfilled the inclusion criteria of Glasgow Coma scale (GCS) were of less than 6 for more than 6 hours Defervescence time were 2 to 3 hours after start of treatment Strength of our study is that patients traditional prompt response, with coma multi-organ dysfunction tends to recover and discharge in 4 days. Result: There were total of 78 patient. Out of them 32 (41.0%) were positive for Malaria with 29 (37.1%) were positive for Plasmodium Falciparum and 3 were having Plasmodium Vivax .all were offered treatment with 57 (73.0%) were given Quinine infusion and 21 (26.94%) were treated with artemisinin infusion. Cure rate was 44 (56.4%) with 46 (58.9%) in quinine group and 14 (17.9%) artemisinin group and (19.2%) and 07 (8.9%) respectively making total mortality of 18 (23.0%). There were 9 (11.5%) patients who left against the medical advice. Average age was 26 ±, majority were male. In addition laboratory derangements like alanine amino transferases (ALT), bilirubin, creatinine, electrolytes as potassium and arterial blood gases(ABGs) were also considered. It has been estimated that recovery time of patients was maximum of 72 hours (time of discharge) for both artimisinin and Quinine dihydrochloride. Late responders were also observed. Occasionally persistent of treatment is needed with no fear of drugs resistance. Conclusion: Quinine infusion is generally safe and effective conventional treatment option, whose benefits and acceptance is well known and we are also documenting this. There has been progress towards decreasing malaria prevalence globally; however, Plasmodium vivax has been less responsive to elimination efforts compared with Plasmodium falciparum . P. vivax malaria remains a serious public health concern in regions where it is the dominant species (South and South-East Asia, the Eastern Mediterranean region, and South America) and is increasingly recognized for its contribution to overall morbidity and mortality worldwide. The incidence of P. vivax decreases with increasing age owing to rapidly acquired clinical immunity and there is a disproportionate burden of P. vivax in infants and children, who remain highly vulnerable to severe disease, recurrence, and anemia with associated developmental impacts. Diagnosis is sometimes difficult owing to the sensitivity of diagnostic tests to detect low levels of parasitemia. Additionally, the propensity of P. vivax to relapse following reactivation of dormant hypnozoites in the liver contributes to disease recurrence in infants and children, and potentiates morbidity and transmission. The 8-aminoquinolines, primaquine and tafenoquine, provide radical cure (relapse prevention). However, the risk of hemolysis in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency necessitates testing prior to administration of 8-aminoquinolines, which has limited their uptake. Additional challenges include lack of availability of pediatric dose formulations and problems with adherence to primaquine owing to the length of treatment recommended. A paucity of data and studies specific to pediatric P. vivax malaria impacts the ability to deliver targeted interventions. It is imperative that P. vivax in infants and children be the focus of future research, control initiatives, and anti-malarial drug development.
