Clinical and angiographic outcomes of patients undergoing entrapped guidewire retrieval in stent‐jailed side branch using a balloon catheter

Abstract: Although the decision to apply the present method for entrapped guidewire retrieval should be made with careful consideration, it appears effective for bail-out. © 2013 Wiley Periodicals, Inc.

“…Irrespective of the occurrence of SB occlusion, jailed wire might be trapped after MB stenting. Although wire entrapment can be detected by the failure in manually pulling out the failed wire, accompanied with a sign of deep intubation of the guiding catheter and was observed in 6.9% (28/406) , the jailed wire might be fractured during withdrawal . This clearly shows jailed wire technique is not a completely safe technique.…”

“…A knuckled tip of a guidewire may lead to distortion of the stent during retraction even if it is in a main branch ( 1 , 2 ). When a jailed wire put up resistance during retraction, using a small balloon or a microcatheter may be useful, causing a diminutive deformation of the stent that can subsequently be fixed by post-dilatation ( 3 ). There are also several stent retrieval techniques described, such as the low-pressure balloon inflation technique, small-balloon technique, double-wire technique, and loop snare technique, which can help to take the distorted or dislodged stent back into the guiding catheter ( 4 , 5 ).…”

“…Next, a 1:1 sized balloon in the MV stent should be underinflated to protect the MV stent when the trapped wire is pulled. Sakamoto et al [16] reported successfully retrieving a stent-jailed SBW by using a balloon catheter without a protective balloon in the main stent. However, Gagnor et al [3] described unintentionally removing the stent when removing a fractured guide wire.…”
