Devastating Consequences of a Jailed Knuckled Wire in CTO PCI of an Anomalous RCA

Abstract: A knuckled tip of a jailed guidewire may lead to distortion of the main branch stent when pulled back after balloon dilatation of the side branch ostium. The distorted stent can be crushed to the side of the vessel as a bail-out intervention. ( Level of Difficulty: Advanced. )

“…This method has the advantages of high sensitivity, high specificity, low contamination, and stable reaction. Possible reasons for this are, on the one hand, the low efficiency of SAT assay using the traditional boiling lysis method for nucleic acid extraction, coupled with the high number of inhibitors of SAT assay reactions in the urine composition, resulting in the insensitivity of SAT assay to urine and affecting nucleic acid ampli-fication [25]. In contrast, the isolated culture method, which is the gold standard for the diagnosis of gonorrhea pathogenic bacteria, has high requirements for the environment, collection, and transport of specimens and also may lead to missed detection due to antibiotic use and inactivation of pathogenic microorganisms after inoculation [26].…”

“…We had successful PCI in 4 out of 5 CTO cases, 2 of which even required a rotablation for the balloon-uncrossable, calcified CTO lesion. Though transfemoral access is the preferred route for PCI 3,4,6,7,9 , transradial access was also attempted by Lin et al 6…”
