Clear cell syringofibroadenoma (of Mascaro) of the nail

Abstract: Eccrine syringofibroadenoma (ESFA) is a rare disorder. We report the first case of ESFA of the nail apparatus, which presented as a yellow longitudinal onycholytic band of the left fourth finger over an intermittently painful subungual filamentous tumour. Histological examination showed features of ESFA with a digitate pattern of papillomatosis due to the specialized physiological longitudinal arrangement of the ridges in the nail bed. In addition, we describe a new feature of colloidal iron-positive clear cel… Show more

“…There is typically an intimately associated fibrovascular stroma and inflammatory infiltrate including prominent mast cells and plasma cells. Similar to CCA, ultrastructural analysis has revealed the presence of accumulated glycogen within keratinocytes, although a ‘clear cell’ variant of ESFA has been relatively rarely reported . Immunohistochemical analysis of ESFA has shown a pattern that most closely resembles the dermal portion of the eccrine duct .…”

“…Spiradenomas can rarely be associated with Brooke-Spiegler syndrome, a rare autosomal dominant inherited disorder, characterised by the development of multiple skin adnexal tumours. 40 Microscopically, they involve a well-circumscribed nodular tumour in the dermis and subcutaneous tissue, Richly vascular stroma with occasional thrombi, foci of squamous differentiation, fibrosis and cystic degenerative changes are commonly seen. Apocrine differentiation has also been reported in spiradenomas, leading some authors to suggest that not all spiradenomas are of eccrine origin.…”

“…The secondary nail involvement that was seen has been rarely reported. [ 12 13 ] The lesion was subsequently excised by the general surgeon and there was no evidence of any recurrence during 1-year follow up. There are reports of carcinomatous transformation of ESFA; hence, every case should be followed in view of this risk of developing carcinoma.…”
