Cisplatin and Etoposide in Childhood Germ Cell Tumor: Brazilian Pediatric Oncology Society Protocol GCT-91

Abstract: The improvement in survival by the introduction of a standard protocol is an important achievement. This is of particular importance for smaller institutions with previous limited experience in the treatment of childhood germ cell tumors. In addition, the results of a two-agent regimen with PE were favorable (5-year overall survival rate is 83.3% for patients in the high-risk group [n = 36] who received PE v 58.8% for patients in the high-risk patients group who received PE plus ifosfamide, vinblastine, and bl… Show more

“…The predominance of advanced disease and non-SE histologies in our series differs from the classical literature, where seminomas and stage I disease is the most common finding [33]. Our hospital is a reference center for TGCT, associated to the Brazilian Childhood Germ Cell Tumor Study Group, a consortium developed to standardize diagnostic assessment and multidisciplinary treatment of TGCT patients in Brazil [34] and for that reason, our series might be biased by more advanced cases. However, the several risk groups were well represented in the study.…”

“…A recent study from the Osteosarcoma Brazilian Group indicated an overall survival of 60% for patients with nonmetastatic disease; a large tumor size was observed in 43% of the cases and was not associated with longer lag time (23). After the introduction of a standard protocol, the five-year overall survival for high-risk patients with germ-cell tumors was 83.3%, which is particularly relevant to the treatment protocols in small institutions with limited experience in the treatment of this rare disease (24). …”

“…With regard to the therapeutic strategies, previous experts summarized their experiences including surgery, chemotherapy, radiotherapy, and high-dose chemotherapy with stem cell support, using small samples (Rodney et al 2010; Kang et al 2008; Bokemeyer et al 2001; Lopes et al 2009; Sakurai et al 2004; Bokemeyer et al 2003; Mann et al 2000). In this study, patients were categorized into four groups: those who received three treatments surgery, chemotherapy, and radiotherapy irrespective of the sequence, those who received two treatments chemotherapy plus surgery or radiotherapy, and those who received single treatment chemotherapy or surgery.…”
