Background: Vitamin D deficiency is widely speculated to be associated with prostate cancer (PCa) incidence, progression, aggressiveness, and metastatic potentials. However, evidence of this is limited among the black population. Hence, this study was spurred by the dearth of data in this regard. Methods: This was a prospectively designed/executed casecontrolled descriptive study carried out in the University of Port Harcourt Teaching Hospital (UPTH) in the Niger Delta subregion of Nigeria. Serum Vitamin D level status was determined/compared between the 380 histologically-verified positive PCa cases and the smoking/sex-matched 380 histologically-verified negative controls using descriptive and comparative statistical tools. The relationship/association between PCa grade/stage and Vitamin D level status was ascertained using crude and adjusted regression models. Data were managed and analyzed with the Statistical Package for Social Sciences version 23 and a p-value of <0.05 was deemed statistically significant. Results: The histologically-verified positive PCa patients had significantly lower mean Vitamin D level status (PCa patients: 24.55 ± 3.47 vs. controls: 49.73 ± 4.08; p<0.001) but higher mean prostate volumes, BMI status, plasma intact PTH levels, and total PSA levels compared to the histologically-verified negative controls. A decreasing trend of serum Vitamin level status was observed with worsening/increasing PCa grade and stage (p<0.05) among the biopsy positive PCa cases. An inverse relationship existed between Vitamin D level status and PCa grade/stage among the Vitamin D deficient PCa subgroup (p<0.05) but not the sufficient/insufficient PCa subgroups (p>0.05). Among the Vitamin D deficient PCa patients, this inverse relationship continued to strengthen with worsening PCa grade/stage. When compared with the PCa patients with the lowest PCa grade (ISUP grade 1) and stage (T1), an increased likelihood of Vitamin D deficiency was significantly associated with worsening PCa grade (ISUP 2 to 5) and stage (T2 to T4) on crude multiple logistic regression model which was subsequently amplified following adjusting for observed confounders. Conclusion:The study findings corroborate the epidemiologic evidence of the association of Vitamin D deficiency with PCa grade and stage; factors that define PCa aggressiveness and metastatic potentials. However, more robust studies among populations of the black race are highly recommended to validate conclusions from this current study.

Zusammenfassung Hintergrund Die Androgendeprivationstherapie (ADT) spielt in der Behandlung des fortgeschrittenen Prostatakarzinoms eine zentrale Rolle. Der zusätzliche Einsatz neuer Medikamente führt sowohl in der hormonsensitiven Situation als auch der Kastrationsresistenz zu einem verlängerten Gesamtüberleben. Ein dadurch bedingter, langjähriger Einsatz der ADT rückt mögliche Komplikationen in den Vordergrund. Dies gilt insbesondere für kardiovaskuläre Ereignisse. Ziel der Arbeit Das Ziel der Arbeit war die Prüfung der aktuellen Datenlage zu möglichen Unterschieden des kardiovaskulären Risikoprofils von Gonadotropin-Releasing-Hormon- (GnRH-)Agonisten und GnRH-Antagonisten. Methoden Narrativer Bericht basierend auf einem Expertenkonsens, unterstützt von einer Literaturrecherche in PubMed (MEDLINE) und den Abstract Datenbanken von ASCO und ESMO zwischen Januar 2015 und 2021. Berücksichtigt wurden für den Behandlungsalltag bedeutsame Metaanalysen, vergleichenden randomisierte klinische Studien (RCT) und „real world data“ (RWD). Die Studienauswahl wurde hinsichtlich der klinischen Relevanz für den Praxisalltag vorgenommen. Ergebnisse Es wurden drei für die Thematik relevante Metaanalysen, zwei prospektive RCT sowie drei RWD-Publikationen identifiziert. Dabei zeigt sich übereinstimmend ein Vorteil für GnRH-Antagonisten mit einer geringeren Inzidenz kardiovaskulärer Ereignisse im Vergleich zu GnRH-Agonisten. Lediglich eine RWD-Untersuchung berichtet über eine vergleichbare Komplikationsrate mit beiden Substanzgruppen. Schlussfolgerung Die GnRH-Antagonisten weisen ein geringeres Risiko für das Auftreten kardiovaskulärer Ereignisse als GnRH-Agonisten auf. Eine Risikominimierung sollte durch Berücksichtigung bekannter kardiovaskulärer Risikofaktoren vor Therapieeinleitung vorgenommen werden.

Objective: Due to inconsistent results in earlier investigations regarding the relationship between vitamin D and prostate-specific antigen (PSA), this study was conducted to gain a deeper understanding of the association between vitamin D and PSA. Methods: A total of 7174 male samples with 25(OH)D, PSA, and other variables were obtained from the National Health and Nutrition Examination Survey (NHANES) database. Three models, created through stepwise logistic regression, were employed to examine the dose-response association between PSA and 25(OH)D. Subsequently, restricted cubic spline analysis (RCS) was used to explore the nonlinear association between 25(OH)D and PSA. The study also compared the performance of four machine learning models in predicting PSA levels. Results: The dose-response relationship indicated a negative impact of high 25(OH)D levels on PSA (p for trend 0.05). The odds ratio (OR) of Q4 (7.73 with 95% CI (0.26, 15.76)) was significantly higher than Q1 (6.23 with 95% CI (0.24, 12.57)). OR values in Q2 and Q3 were less than 1 (Q2= 0.57 with 95% CI (-6.37, 8.04) and Q3= 0.26 with 95% CI (-5.94, 6.86)), suggesting a potential protective effect of 25(OH)D on PSA. RCS analysis revealed a U-shaped relationship between blood 25(OH)D levels and PSA, with serum 25(OH)D in the range of 20-134 ng/ml showing a potential decrease in PSA levels. Above this range, an increase in 25(OH)D might elevate PSA levels. Age (2.67 with 95% CI 2.24 to 3.1) and BMI (17.52 with 95% CI 7.65 to 26.32), along with the OR of obesity (10.36 with 95% CI 0.68 to 20.18), were identified as potential PSA risk factors. Among the machine learning models, the random forest algorithm performed the best in predicting PSA levels. Conclusion: This study revealed a U-shaped relationship between 25(OH)D and PSA, with PSA potentially declining when 25(OH)D is between 20 and 134 ng/mL and possibly rising above this range. The random forest method proved effective in both predicting PSA levels and guiding vitamin D dosage.
