Circulating MicroRNAs as Novel Biomarkers in Risk Assessment and Prognosis of Coronary Artery Disease

Abstract: Coronary artery disease is among the leading causes of death worldwide. Nevertheless, available cardiovascular risk prediction algorithms still miss a significant portion of individuals at-risk. Thus, the search for novel non-invasive biomarkers to refine cardiovascular risk assessment is both an urgent need and an attractive topic, which may lead to a more accurate risk stratification and/or prognostic score definition for coronary artery disease. A new class of such non-invasive biomarkers is represented by … Show more

“…Asymptomatic course and progression make an early diagnosis of CAAD difficult. miRNAs play a role in the mechanism of atherosclerosis, and atherosclerotic coronary artery disease is characterized by specific signatures of miRNA in tissue and plasma assays [ 23 , 24 , 25 , 26 ]. However, scarce data concerning miRNAs involved in CAAD pathology prompted us to study five circulating miRNAs: MiR-125b-5p, miR-210-3p, miR-328-3p, miR-425-3p, and miR-483-5p, which we preselected and described in our former study [ 14 ] as potentially associated with CAAD.…”

“…Since the presence of miRNAs in plasma was first mentioned in the literature, circulating miRNAs have been reported as a potential biomarker of heart failure and CAD [ 23 , 28 ]. In contrast to the numerous studies in CAD cohorts, the predictive value of miRNAs in CAAD is yet to be discovered.…”

“…To date, multiple miRNAs related to chronic coronary syndromes have been identified, being proposed as biomarkers with potential clinical applications in diagnosis and prognosis. However, the miRNA expression shows wide variability in the literature [ 24 , 37 , 38 ]. To avoid this methodological problem, we will relate the different coronary patterns only to those circulating miRNAs showing consistent results in the different research works, and we will perform specific analyses to take into account the potential variability on miRNA levels between centres, since this is a multicentric study.…”

“…In addition, miRNAs play a significant role in lipid metabolism beyond cholesterol, and also in insulin signalling in the liver, thus contributing to the pathways that are often dysregulated in metabolic disorders, such as non-alcoholic fatty liver disease (NAFLD) and type 2 diabetes mellitus (T2DM), which are closely linked to CAD 6 . MicroRNAs have been proposed as regulators and biomarkers of CAD pathogenesis but only few of these have been shown in several studies to be consistently up- or down-regulated in stable or acute CAD 7 . Variable results in miRNA studies are most probably due to factors like differences in miRNA detection methods, variable sample source as well as variable CAD criteria.…”
