Circadian Variation of Migraine Attack Onset: A Review of Clinical Studies

Abstract: Several studies suggested that migraine attack onset shows a circadian variation; however, there has not been an overview and synthesis of these findings. A PubMed search with keywords “migraine” AND “circadian” resulted in ten studies directly investigating this topic. Results of these studies mostly show that migraine attacks follow a monophasic 24-hour cyclic pattern with an early morning or late night peak while other studies reported an afternoon peak and also a biphasic 24-hour cycle of attacks. The iden… Show more

“…The pathological mechanism underlying the association between migraine and insomnia is not yet fully understood. According to most of the available studies, migraine attack onset follows a circadian variation, with an early morning or late-night peak of migraine attack onset [ 54 ]. The observed circadian pattern of migraine attack onset may be related to a temporal relationship with rapid eye movement (REM) sleep stages.…”

“…Symptoms from a part of the nervous system also follow circadian modulation. Migraine attacks and mood worsening in patients suffering from major depressive disorder occur predominantly in the morning; however, afternoon and evening episodes are also reported (26)(27)(28). Exacerbation of behavioral symptoms, socalled sundowning, in some of Alzheimer disease patients takes place during the afternoon and in the evening and is related to a phase delay of the body temperature rhythm (29)(30)(31).…”

“…Fewer reports describe biphasic patterns [ 6 ], or a peak around the middle of the day [ 1 ]. Those contradictory results have been reviewed by Bkasa et al, [ 7 ] who highlight the methodological differences between the studies that may contribute to the differences (number of patients, prospective or retrospective nature, specific populations, type of headache, use of medication). Recently, a study showed that migraine patients have a higher likelihood to be “morning larks” (i.e.…”
