The treatment of chronic pain with cannabinoids is becoming more widespread and popular among patients. However, studies show that only a few patients experience any benefit from this treatment. It also remains unclear which domains are affected by cannabinoid treatment. Therefore, the present study is novel in that it explores the effects of cannabinoid treatment on four patient-related outcome measures (PROMs), and includes patients with chronic refractory pain conditions who have been given the option of cannabinoid treatment. A retrospective design was used to evaluate the impact of cannabinoid treatment on patients with refractory pain in two German outpatient pain clinics. The present study shows that pain intensity (mean relative reduction (−14.9 ± 22.6%), emotional distress (−9.2 ± 43.5%), pain-associated disability (−7.0 ± 46.5%) and tolerability of pain (−11 ± 23.4%)) improved with cannabinoid treatment. Interestingly, the trajectories of the PROMs seemed to differ between patients, with only 30% of patients responding with respect to pain intensity, but showing improvements in other PROMs. Although the mean treatment effects remained limited, the cumulative magnitude of change in all dimensions may affect patients’ quality of life. In summary, a singular evaluation with pain intensity as the sole outcome does not cover the multidimensional effects of cannabinoids. Therefore, the treatment effects of cannabinoids should be evaluated with different PROMs.

Objective Negative body image is an increasingly important factor in chronic pain disorders; particularly because the two conditions mutually influence each other. Our study examines body-image-related attitudes and comorbid psychic symptoms in patients with chronic pain disorders. Methods and Measures 188 patients with chronic pain answered the Dresden body image questionnaire (DKB-35), the Hospital Anxiety and Depression Scale (HADS) and the Beck’s Depression Inventory (BDI). Results The mean values of the DKB-35 subscales range between 2.24 und 3.29. The subscale “vitality” has the lowest mean value. BDI and HADS correlated significantly with the DKB-35 subscales. The subscale “body-acceptance” intercorrelated high with the two subscales “vitality” and “self-aggrandizement”. Conclusion Our findings verify that chronic pain is strongly associated with negative body image. The issues of vitality and psychological symptoms seem particularly central in this complex interaction. The biopsycho-social model and the avoidance-endurance approach to chronic pain offer important ideas for explanation and treatment.

Krankenhäuser haben im Rahmen ihres Angebots für eine interdisziplinäre multimodale Schmerztherapie (teilstationär und stationär) immer wieder mit strengen Detailprüfungen der zu kodierenden Prozedurenkodes (OPS 8‑918.xx; 8‑91c) durch Krankenkassen und den Medizinischen Dienst zu kämpfen. Dabei werden regelmäßig die Notwendigkeit der (teil-)stationären Behandlung im jeweiligen Sektor, dokumentierte Therapieanteile und die Qualifikation der Therapeuten überprüft. Das Bundessozialgericht hat am 27.10.2020 zur konkreten Qualifikation von psychologischen Psychotherapeuten (BSG vom 27.10.2020, Az.: B 1 KR 25/19 R) entschieden. Das Urteil und seine potenziellen Auswirkungen werden in der vorliegenden Übersicht erläutert und diskutiert.
