Chronic traumatic encephalopathy: A paradigm in search of evidence?

Abstract: Chronic traumatic encephalopathy (CTE) has been in the medical literature since the 1920s. It is characterized clinically by diverse neuropsychiatric symptoms, and pathologically by variable degrees of phosphorylated tau accumulation in the brain. The evolving paradigm for the pathogenesis of CTE suggests that concussion or subconcussion from athletic participation initiates a cascade of pathologic events, encompassing neuroinflammation and protein templating with trans-synaptic neurotoxicity. The end result i… Show more

“…1 The high incidence of concussion and all-cause injury in football is of concern, considering the large number of participants and potential for hazardous long-term outcomes. 5,6 Establishing injury rates in football is important to assess current-and inform future-injury prevention initiatives, monitor injury trends, and better inform participants of the potential risks of participation.…”

“…Further, it is argued that there is "no prospective scientific basis that can be used to support [the] link" between brain pathology and symptomology (Gaetz, 2017, p.140. Emphasis in original) and, indeed, that drawing such a link is fundamentally beyond the scope of these neuropathological studies (Castellani, 2015). Moreover, the hypothesis that this particular form of neurodegenerative dementia has a specific identifiable cause contrasts with the multifactorial aetiology associated with other cognitive and behavioral conditions (Gaetz, 2017).…”
