Chronic pelvic pain (CPP) has many potential causes and is often a complex disorder with multiple contributing etiologies. The evaluation and treatment of women with CPP often requires a multidimensional approach. The treatment of CPP consists of two approaches: 1) treatment of pain itself or global treatment, and 2) treatment of disease-specific etiologies. Most often, treatment requires a combination of both approaches. This article reviews recent literature in the global treatment of CPP, including pharmacologic, psychotherapy, and neuroablative, as well as specific interventions for endometriosis, interstitial cystitis, pelvic adhesive disease, adenomyosis, and pelvic venous congestion.

Myofascial pain as a cause of chronic pelvic pain with or without pelvic organ pathology is well-documented in the literature. Causes of this pain are multifactorial, including specific pelvic organ pathologies, neuromuscular disorders, and psychologic causes. Management of this myofascial component of chronic pelvic pain involves a multidisciplinary approach including physicians, physical therapists, neurologists, and psychiatrists. Treatment strategies, including behavioral management, medications, physical therapy, trigger point injections, neuromodulation, botulinum toxin injection, and other lesser known treatment modalities, are discussed in detail in this article.

informació n del artí culo Objetivos: Dar a conocer su existencia a los profesionales de la urología a travé s de una revisió n sistemá tica de la bibliografía existente y sus posibilidades de diagnostico y tratamiento.Material y método: Realizamos una bú squeda bibliográ fica a travé s de la base de datos )Pubmed* utilizando los té rminos )Pudendal nerve*, )Pudendal nerve entrapment síndrome*, )Pelvic floor neuropathic pain*, )Pelvic floor myofascial síndrome*, )Pudendal nerve decompression*. Asimismo, seleccionamos los trabajos en lengua inglesa, españ ola y francesa, revisando tambié n los artículos que dichos trabajos refieren.Resultados: Se desarrollan los aspectos má s destacados del síndrome: anatomía, etiología, fisiopatología, diagnostico clínico, ecodoppler de la arteria pudenda interna, estudios electrofisioló gicos, criterios diagnó sticos, terapé utica mé dica, tratamiento con fisioterapia y tratamiento quirú rgico.Conclusió n: Es una causa poco frecuente de dolor en el suelo pé lvico y afecta en gran medida la calidad de vida de los pacientes. Hoy, disponemos de herramientas diagnó sticas y terapé uticas que nos permiten tratar este síndrome invalidante con buenos resultados.
