Objective: This study aimed to compare hearing and anatomical outcomes after ossicular chain reconstruction with titanium or hydroxyapatite prostheses. Methods:In this study, patients who underwent tympanoplasty and ossicular chain reconstruction with titanium or hydroxyapatite prostheses at a university hospital from January 2007 to February 2013 were retrospectively reviewed; they had a minimum follow-up period of 6 months. Patients were divided into 4 groups according to the type of prostheses. The surgical procedure, follow-up examinations, preoperative, and postoperative audiometry results were noted and evaluated for partial and total prostheses. The results were compared both for titanium and hydroxyapatite prostheses. Results:The study subjects included 51 patients. Titanium had better hearing results in partial prostheses (p<0.05), while the anatomical outcomes were similar. Nevertheless, both types had similar results in total prostheses (p>0.05). ÖzetAmaç: Kronik otitis media tanısı ile opere edilen hastalarımızın erken dönem sonuçlarını gözden geçirmek. Gereç ve Yöntem: Ocak 2011-Ocak 2012 tarihleri arasında kronik otitis media tanısı ile tip 1 timpanoplasti, tip 3 timpanoplasti, modifiye radikal mastoidektomi yapılan 49 hasta retrospektif olarak değerlendirildi. Ameliyat esnasındaki yaşları, zardaki perforasyon tipi (santral, attik, subtotal), greft başarısı ve pre operatif ve post operatif 3. aydaki saf ses odyometri sonuçları değerlendirildi. Bulgular: Çalışmaya dahil edilen 49 hastanın post operatif takiplerinde; 39 hastada greft zar intaktı, 10 hastada reperforasyon izlendi. Saf ses odyometri sonuçlarına göre; tip1 timpanoplasti uygulanan ve temporal kas fasyası kullanılan 23 hastada ortalama 7.7 dB, tragal kartilaj grefti kullanılan 18 hastada ortalama 11.9 dB, tip 3 timpanoplasti uygulanan 3 hastada 14.3 dB, modifiye radikal mastoidektomi uygulanan 5 hastada 21.4 dB kazanç sağlandığı tespit edildi. Sonuç: Timpanoplasti uygulanan hastaların, yaş, cinsiyet, perforasyon yeri, ameliyat tekniği , postoperatif erken dönemde işitme kazanç sonuçları değerlendirildi. Material and Method: 49 chronic otitis media diagnosed patients who underwent type 1 tympanoplasty, type 3 tympanoplasty and modified radical mastoidechtomy between January 2011 and January 2012 were examined retrospectively. The age during the surgery, perforation types of eardrum (central, attik, subtotal), graft success, and pre-operative and post-operative, in the 3rd month, pure tone odiometry results were evaluated. Results: 49 patients were included. Intact with 39 patients and reperforation with 10 patients were observed. Based on pure tone odiometry results; in 23 patients, who underwent type 1 tympanoplasty and used temporal fascia autografts, an average of 7.7 dB; in 18 patients, who used tragal cartilage graft, an average of 11.9 dB; in 3 patients, who underwent type 3 tympanoplasty, an average of 14.3 dB and in 5 patients, who underwent modified radical mastoidechtomy, an average of 21.4 dB hearing gain was observed. Discussion: In early post-operative period, of patients who underwent Tympanoplasty, age, gender, perforation location, operation technique, and hearing gain results were assessed.

OBJECTIVE:Chronic otitis media (COM) is a disorder characterized by perforation of the eardrum and hearing loss following chronic inflammation of the middle ear cavity, ossicules, and mastoid cells. Eustachian dysfunction plays an important role in COM etiopathogenesis and postoperative prognosis. The determinants of postoperative prognosis are still being researched. This study aimed to research the prognostic value of acoustic rhinometry (ARM) and rhinomanometry (RMM) in COM surgery in terms of eradication of the infection after operation, graft success, and hearing gain in operated cases. MATERIALS and METHODS:This study included 58 patients who underwent surgery with a diagnosis of COM. Patients were assessed in terms of age, gender, COM type, treatment methods used, eradication of infection, graft success, and hearing gain. ARM and RMM measurements were performed in the preoperative period. ARM and RMM values were statistically compared in terms of the existence of postoperative infection, graft success, and hearing gain. RESULTS:In terms of ARM and RMM measurements, there was no statistically significant difference between cases where postoperative infection control was assured and cases with ongoing infection; successful and failed cases in terms of grafting; or successful and failed cases in terms of postoperative hearing. When preoperative and postoperative air-bone gap averages were compared, statistically significant differences were observed. CONCLUSION:In the presence of a nasal obstruction in cases with chronic otitis, elimination of this situation is the first line of treatment. Infection control, graft success, and improvement of hearing will be possible to a greater extent in the postoperative period for patients with the nasal pathology remedied.
