Samenvatting In dit artikel wordt een onderzoek beschreven naar de effecten van een revalidatieprotocol, dat bestaat uit geleidelijke exposure in vivo en een geleidelijk activiteitenprogramma bij een patie¨nte met chronische lagerugpijn die bang was voor (hernieuwd) letsel. Er is gebruikgemaakt van een ABCsingle-case design, waarbij een baseline-fase, het activiteitenprogramma en geleidelijke exposure elk drie weken in beslag namen. Met behulp van een draagbare bewegingssensor is tijdens de laatste week van elke fase de fysieke activiteit thuis gemeten. Pijngerelateerde cognities en angst werden dagelijks gemeten met behulp van VAS-schalen. Bij aanvang van de baseline en na afloop van elk van de drie fasen werd de angst voor bewegen of hernieuwd letsel gemeten. Ook de neiging om in catastrofes te denken, de ervaren beperkingen, pijncontrole en pijnintensiteit werden op die meetmomenten onderzocht. Fysieke activiteit bleek gedurende het activiteitenprogramma en de geleidelijke exposure hoger te zijn dan tijdens de baseline. Pijngerelateerde angst en cognities verbeterden alleen tijdens de geleidelijke exposure. Afname van de angst voor bewegen of (hernieuwd) letsel hing samen met afname van het denken in catastrofes, ervaren beperkingen en pijnintensiteit en met een toename in pijncontrole. Zelfrapportage liet zien dat geleidelijke exposure bij deze rugpijnpatie¨nte met angst voor (hernieuwd) letsel zorgde voor angstreductie en dat dit een positieve invloed uitoefende op de ervaren functionele beperkingen. Als de fysieke activiteit thuis met een ambulante bewegingssensor wordt gemeten, dan lijkt het activiteitenprogramma al te zorgen voor een toename van activiteiten.
