ObjectiveThis study investigated what is important in care delivery from the perspective of hospital inpatients with complex chronic disease, a currently understudied population.Participants and SettingOne‐on‐one semi‐structured interviews were conducted with inpatients at a continuing care/rehabilitation hospital (n = 116) in Canada between February and July 2011.DesignThe study design was mixed methods and reports on patient characteristics and care delivery experiences. Basic descriptive statistics were run using SPSS version 17, and thematic analysis on the transcripts was conducted using NVivo9 software.ResultsPatients had an average of 5 morbidities and several illness symptoms including activity of daily living impairments, physical pain and emotional disturbance. Three broad themes (each with one or more subthemes) were generated from the data representing important components of care delivery: components of the care plan (a comprehensive assessment, supported transitions and a bio‐psycho‐social care package); care capacity and quality (optimal staff to patient ratios, quicker response times, better patient–provider communication and consistency between providers) and the patient–provider relationships (characterized by respect and dignity).ConclusionsAs health systems throughout the industrialized world move to sustain health budgets while optimizing quality of care, it is critical to better understand this population, so that appropriate metrics, services and policies can be developed. The study has generated a body of evidence on the important components of care delivery from the perspectives of a diverse group of chronically ill individuals who have spent a considerable amount of time in the health‐care system. Moving forward, exploration around the appropriate funding models and skill mix is needed to move the evidence into changed practice.

Objectives: over the past few years, lifestyle modification has increased the prevalence of major components of metabolic syndrome (MetS), leading to increased risk of non-communicable diseases, especially cardiovascular disease. The present study was aimed to determine the prevalence of MetS and its components and present results according to the major demographic characteristics of the participants in a population-based study. Methods: This cross-sectional study was conducted on a total of 10663 subjects aged 40-70 years who participated in phase one of PERSIAN Kharameh cohort carried out between 2014 and 2017. Result: According to IDF, ATP III, and an Iranian definition, the prevalence of MetS were reported as 37.00%(36.08-37.92), 33.82% (32.93-34.73), and 33.13% (32.24-34.03), respectively. The most prevalent component of MetS was abdominal obesity (73.59), followed by reduced HDL cholesterol levels (44.83), elevated triglyceride (28.3), fasting glucose (35.34), and high blood pressure (32.64). In addition, the prevalence of MetS were 50.3, 70.4, 68.2, 65.7, and 60.8 among individuals with abdominal obesity, high triglyceride levels, hypertension, impaired fasting glucose, and low HDL-C levels, respectively. Results showed that 90% of the subjects displayed at least one component of MetS, and all MetS components were associated with major demographic characteristics of the study subjects. Conclusions: The findings of this study indicate high prevalence of metabolic risk factors among study population. This study with a large sample size would contribute to establishment of efficient interventions and programs aimed at reducing the prevalence of MetS by health staff, supervisors and policymakers.

Kratak sadržajUvod. Nejednakosti u korišćenju usluga zdravstvene zaštite postoje širom svijeta. Cilj rada je ispitivanje uticaja demografskih, socijalno-ekonomskih nejednakosti, nejednakosti u stilu života i zdravstvenom stanju na korišćenje usluga doktora porodične medicine i specijaliste u Republici Srpskoj, Bosna i Hercegovina.Metode. Ova studija je dio Nacionalnog istraživanja zdravlja stanovništva Republike Srpske provedenog 2010. godine. Od 4673 osobe uzrasta 18 i više godina identifikovane u anketiranim domaćinstvima, 4128 je intervjuisano (1906 muškaraca i 2222 žena). Multivarijantna logistička regresiona analiza je korišćena za procjenu uticaja različitih varijabli na korišćenje usluga primarne i specijalističke zdravstvene zaštite.Rezultati. U poređenju sa muškarcima, viši procenat žena je koristio usluge doktora porodične medicine [unakrsni odnos (UO) = 1,34; 95% interval povjerenja (95% IP) = 1,13-1,60], dok je suprotno kada je u pitanju korišćenje usluga doktora specijaliste (UO = 0,62; 95% IP = 0,49-0,77). Ljudi sa nižim obrazovanjem su posjećivali doktora porodične medicine rjeđe nego visoko obrazovani. Nema značajnih socijalnih nejednakosti u korišćenju usluga doktora porodične medicine i specijaliste u Republici Srpskoj. Osobe sa hroničnim nezaraznim oboljenjima u poređenju sa onima bez ovih oboljenja su češće koristile usluge doktora porodične medicine (UO = 3,90; 95% IP = 3,22-4,73). Muškarci i žene koji procjenjuju svoje zdravlje kao dobro su koristili usluge doktora porodične medicine i specijaliste značajno rjeđe u odnosu na one koji procjenjuju svoje zdravlje kao loše.Zaključak. Postoje značajne demografske, socijalno-ekonomske nejednakosti, kao i nejednakosti u životnom stilu i zdravstvenom stanju osoba koje koriste usluge doktora porodične medicine i specijaliste u Republici Srpskoj. Smanjivanje nejednakosti u korišćenju zdravstvenih usluga u Republici Srpskoj je prioritet u budućnosti.Ključne riječi: korišćenje zdravstvenih usluga, doktor porodične medicine, doktor specijalista, istraživanje zdravstvenog stanja, Republika Srpska
