Chronic Demyelinating Peripheral Neuropathy Associated With Multifocal Central Nervous System Demyelination

Abstract: A series of 6 cases is described in which a chronic demyelinating neuropathy was associated with a relapsing multifocal CNS disorder, the clinical features of which resembled multiple sclerosis. Multifocal CNS lesions were demonstrated by CT and MR imaging and the presence of CNS demyelination was indicated by prolonged central conduction times. These cases are discussed in relation to the occurrence of combined peripheral and central demyelination in chronic relapsing experimental allergic encephalomyelitis a… Show more

“…It may represent peripheral or central effects of the disease and also may precede the onset of limb involvement. 27,[35][36][37] Therefore, the clinician has to be aware of the possibility of CIDP, especially if neuro-imaging reveals cranial nerve hypertrophy, or if Optic neuropathy In a case series of six patients with both CIDP and central nervous system lesions on MRI, three had attacks of optic neuritis with typical clinical courses. Thomas et al, 1987 27 In a case series of six patients with both CIDP and central nervous system lesions on MRI, three had attacks of optic neuritis but clinical details were not reported.…”

“…It is known that peripheral demyelination can accompany demyelination in the central nervous system (CNS) (3,4,5,6,7). In CIDP, symptomatic clinical involvement of the CNS occurs in up to 5% of patients and subclinical abnormalities can be demonstrated in 20% with electrophysiologic studies and magnetic resonance imaging (MRI) (8).…”

“…He mentioned interstitial edemato be an important cause of enlarged nerves, although true interstitial hypertrophy associated with onion bulb formation could develop after recurrent demyelination and remyelination. More recently, Thomas et al presented 4 cases with thickened nerves from among 6 CIDP cases associated with multiple sclerosis (MS)-like central nervous system (CNS) demyelination (5). In 1995, we reported a 9-year-old boy presenting visible posterior auricular nerves under the skin of the lateral sides of the neck (6).…”
