We present a 63-year-old diabetic man with a mid-peripheral pseudo-duplicated optic disk. A large neovascular frond emanated from this structure. Optical coherence tomography showed this frond to be choroidovitreal neovascularization. Magnetic resonance imaging scanning of the globe and orbit was normal with a single optic nerve.
