Chordoma: a case series and review of the literature

Abstract: BackgroundChordoma is a rare malignant tumor of the skull base and axial skeleton, with an incidence of less than 0.1/100,000 per year. Patients with advanced chordoma have a poor prognosis due to locoregional recurrence with infiltration and destruction of surrounding bone and soft tissue. Cytotoxic chemotherapy or other systemic therapies have not been proven to be effective for these diseases. Therefore, several molecularly targeted therapies have been proposed as potentially beneficial, including tyrosine … Show more

“…Controversy over aggressive tumor resection in skull base chordomas arises from the tumor's relationship to critical neurovascular structures and that total resection often results in significant morbidity (2,7,8). Tumor diameter and aggressive growth pattern, previous surgery and radiation therapy have been found to be associated with postoperative morbidity rates (7,8,17,18). Although modern skull base techniques implemented in experienced centers have reduced the mortality and morbidity associated with chordoma resection, chordomas are considered to be tumors that cannot be curable surgically (2,(4)(5)(6)17).…”

“… 4 Cytotoxic drugs are not recommended, however, tyrosine kinase inhibitors (TKI) may help to achieve a stable disease state in patients with widespread or recurrent chordoma who cannot tolerate further surgical or radiotherapeutic intervention. 8 …”

“…Therefore, chordoma resection is associated with a high risk of morbidity and mortality. In addition, the epidemiology of chordoma is unclear ( 9 ), particularly as regards its incidence rates and the most frequently affected parts of the axial skeleton ( 10 , 11 ). This is clinically significant, since the symptoms and treatment strategies of chordoma largely depend on its anatomical location ( 12 , 13 ).…”
