Cholangiocarcinoma: classification, diagnosis, staging, imaging features, and management

Abstract: Cholangiocarcinoma is a relatively uncommon malignant neoplasm with poor prognosis. The distinction between extrahepatic and intrahepatic subtypes is important as epidemiological features, biologic and pathologic characteristics, and clinical course are different for both entities. This review study focuses on the role imaging plays in the diagnosis, classification, staging, and post-treatment assessment of cholangiocarcinoma.

“…11,14 Anatomically, iCCA is defined as a malignancy located in the periphery of the second-order bile ducts; thus it can arise from segmental bile ducts to smaller branches of the intrahepatic biliary tree. 11 pCCA arises in the right and/or left hepatic duct and/or at their junction 15 ; it is clinically also known as Klatskin tumour, 16 although this term should be discouraged. dCCA involves the common bile duct.…”

“…In patients suspected of having CC, imaging plays a critical role in diagnosis and treatment planning (71,72). Since surgical complete resection is the only potentially curative treatment option, selection of the most appropriate surgical candidates and determination of the type and extent of surgery for the selected candidates are the primary goals of imaging.…”

“…Based on the anatomical site, CCAs is classified in two major groups: intrahepatic (iCCA) and extrahepatic (eCCA) CCA [5]. The iCCA is located in the intrahepatic biliary tree [5,6] whereas eCCA may rise in the extrahepatic bile ducts [7,8]. CCA is characterized by very poor outcome, showing an increase in incidence and mortality rate worldwide [9][10][11].…”
