Children with respiratory tract infections in Swedish primary care; prevalence of antibiotic resistance in common respiratory tract pathogens and relation to antibiotic consumption

Abstract: BackgroundThe majority of antibiotics consumed in developed countries are prescribed in primary care. However, little is known about resistance levels in the primary care population.MethodNasopharyngeal cultures were obtained from children, 0-10 years of age, seeking care at their Primary Health Care Centre with symptoms of respiratory tract infection. Parental questionnaires were used to retrieve information about the child’s previous antibiotic consumption.ResultCultures from 340 children were gathered. The … Show more

“…Streptococcus pneumoniae are a common cause of relatively mild localized infections such as otitis media but can also cause more severe infections like pneumonia and live threatening invasive diseases. Penicillin is the antimicrobial of choice for patients infected with susceptible pneumococci [1][2][3][4]. Non-susceptibility to penicillin and other antimicrobials is common in many countries and may necessitate higher doses of penicillin, or broad-spectrum antimicrobial agents [5][6][7].…”

“…11,12 Compared to most other countries, Sweden, the Netherlands, and a few other nations, have a low antibiotic prescription rate and low prevalence of antibiotic resistance in Streptococcus pneumoniae, the most common cause of pneumonia. 13,14 The drug of choice to treat pneumonia in Sweden is phenoxymethylpenicillin, followed by doxycycline. 6 To the authors' knowledge, if or how the management of LRTIs has changed over time has not been explored in a low prescribing country.…”

“…W wyniku działań prowadzonych przez ostatnich 25 lat zużycie antybiotyków w lecznictwie otwartym w Szwecji jest jednym z niższych w Europie, jak również poziom oporności na antybiotyki kluczowych patogenów wywołujących zakażenia jest niższy w porównaniu z innymi krajami. Podjęte działania wpłynęły na ograniczenie przepisywania antybiotyków w pediatrii o 70% [34]. Jest to przykład godny naśladowania.…”
