Background and purpose: The sphenoid sinus is characterized by the frequency of anatomical variants that cause operative difficulties and complications during the transphenoidal approach. These variants involve the geographic factor and the type of skin phenotype. Most of the studies have been carried out on European, North African and Asian populations. Some studies have been found in sub-Saharan Africa but not in Côte d’Ivoire. The aim was to determine, from CT data, the anatomical characteristics of the sphenoid sinus in the Ivorian population. Methods: Cranioencephalic computed tomography images of 100 patients (80 men and 20 women aged 16 to 71) were used for the study. These were patients who presented with head trauma without injury to the base of the skull. Were studied the anatomical characteristics of the sphenoid sinus. Results: The type of pneumatization was sellar in 58% sinuses, pre sellar in 41% and conchal in 1% of cases. The presellar type predominated in women (60%) and the sellar type in men (65.5%). Extensions were present in 66.34% of cases, more frequent in men (70%), in the sellar type (79.31%), and in the anterior clinoid process (46.20%). The procidence of vasculo-nervous structures was more frequent in men (52.62%) and in the sellar type (62.07%), the procidence of the internal carotid artery was bilateral in 30.69% of sinuses, that of the optic nerve was mainly unilateral left (07.92%) more frequent in women ( 10%) and in the sellaire type (09.43%). Dehiscence was present in 15% of cases. Dominance was straight in 63.64% of the sinuses. The Onodi cell was present in 65% of sinuses. The ostium was bilateral in (45%) of the sinuses. Conclusion: The Ivorian sphenoid sinus is highly pneumatized, facilitating access to the surgeon; the sellar type is the most frequent. On the other hand, the frequency of procidence and dehiscence of vasculo-nervous elements increases the risk of surgical complications. Goal: The aim of this work is to study the relationship between the pneumatization of the adjacent structures of sphenoid sinuses and the protrusion of the neurovascular structures in the sinuses. Methods: A review of 225 CT scans skull was done for subjects aged at least 16 years old from November 1st to December 31st 2017. The pneumatization of adjacent structures of sphenoid sinuses and its relation with the protrusion of neurovascular structures surround the sinuses have been investigated. We used Fischer Exact test for comparison. The p value < 0.05 was expressed as significant. Results: Statistically significant associations were found between anterior clinoid process bilateral pneumatization and bilateral protrusion of carotid canal (p < 0.05) and of optic canal (p < 0.001). There were also, statistically significant correlations between bilateral pterygoid process pneumatization and bilateral protrusion of carotid canal and of optic canal (p < 0.001); and between pterygoid process bilateral pneumatization and bilateral protrusion of maxillary and vidian nerves (p < 0.001). An association was also found between bilateral pneumatization of great wings and bilateral protrusion of maxillary nerves (p < 0.001) and of vidian nerves (p < 0.05). Conclusion: Compared to the literature, the prevalence of pneumatization of the adjacent structures of sphenoid sinuses was lower on Beninese than Caucasian and Asian. But there were the same correlations with the protrusion of neurovascular structures. In case of endonasal surgery of the sphenoid sinuses, surgeon should be aware of How to cite this paper:

Are Maxillary and Sphenoid Sinus Volumes Predictors of Gender and Age? A Cone Beam Computed Tomography Study

This study aimed to analyze the applicability of maxillary sinus (MS) and sphenoid sinus (SS) volumes in determining gender and age. Materials and Methods:This retrospective study included cone beam computed tomography images of 164 MSs and 164 SSs of 82 patients (40 females and 42 males). Right, left, and total MS and SS volumes were estimated. The independent samples t-test, logistic regression, and Pearson correlation coefficient were used to explore their accuracy in determining age and gender.Results: Males had greater right SS, average, and total SS volumes than females (p=0.007, p=0.013, and p=0.013, respectively). The right SS volume had the best predictive power (65.9%) for gender. A regression formula using this parameter alone showed satisfactory accuracy. No significant correlations were found between age and MS or SS volumes (p>0.05). Conclusions:The SS volume is a valuable tool in determining gender, whereas neither MS nor SS are reliable predictors of age. A simple gender determination is possible with our regression formula using the right SS volume. Further studies are needed to investigate the role of paranasal sinus volumes with a view to develop formulations in determining age and gender in different populations.
