Or use: https://youtu.be/uIRDqugjJv4 Background and Objective: Disease activity score 28 (DAS28) for rheumatoid arthritis (RA) is the commonly used DAS; it relies on clinical parameters that could be subjective. This work aimed to create a more accurate DAS for RA and assess its validity. Patients and Methods: The study included 98 RA patients and 53 matched controls; they were interviewed, clinically examined, their visual analogue scales (VAS) were reported, and then blood samples were withdrawn for erythrocyte sedimentation rate (ESR), complete blood count (CBC), and C-reactive protein (CRP). Platelet indices (PIs) were obtained from the CBC including Plt (platelet count), mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT). DAS28 was calculated for each patient using RheumaHelper mobile software. Minitab Statistical Package ® and SPSS v20 software were used for data analysis. Results and Conclusions: Results revealed perfect matching between patients and controls as regarding age and gender. ESR, CRP and PDW were significantly higher in patients than controls; also positive correlations were detected among these variables. A new DAS for RA was developed; ESR, CRP, PDW and MPV were the components for this index. Further analyses showed that this new score was significantly higher in patients than controls and correlated with DAS28 of the patients. Furthermore the new score could identify RA patients from healthy subjects (cut off value < −0.79) and stratified RA patients according to their disease activity into low, intermediate, high, or in remission. Conclusively, we developed a more precise, easily obtained new DAS for RA. This new DAS has both diagnostic/prognostic values in patients with RA.

Objective: To investigate the relationship between established acute inflammatory markers in COVID-19 patients with Mean platelet volume (MPV) and Platelet large cell ratio (P-LCR). Materials and Methods: This was a retrospective longitudinal study conducted at Fauji Foundation Hospital, Rawalpindi from 10th June to 10th August 2020. Biochemical parameters i.e. CRP, serum ferritin, LDH, and pro-BNP as well as hematological parameters (MPV and P-LCR) were noted once every week during admission of all the COVID-19 positive patients.Data were analyzed using IBM-SPSS version 23. Repeated measure ANOVA using a generalized linear model was done to check the trend of values during the duration of their stay. Pearson Correlation analysis and regression models were estimated to check the relationship of MPV and P-LCR with C- reactive protein, serum Ferritin, LDH, and pro-BNP. P-values less than 0.05 were considered statistically significant. Results: The mean age of the studied group was 55.47 (SD=±15.0) years with the female to male ratio being 2:1. Mean platelet volume showed a statistically significant positive correlation with the established set of inflammatory markers other than pro-BNP with a p-value of less than 0.05.P-LCR however showed a positive correlation with CRP (p-value of less than 0.05) only with no significant correlation with other biochemical markers. Conclusion: MPV is reported on routine complete blood count report (CBC). It is readily available at even the most under-resourced health centers; therefore reporting the platelet indices does not require extra testing, sampling, or reagent cost. A statistically significant positive correlation amongst the established acute inflammatory markers and relatively understudied platelet indices (MPV) in COVID-19 provides a cost-effective, readily available, and time-efficient tool for marking disease progression.

Background: Thrombocytopenia is one of the most common causes of abnormal bleeding and is defined as platelet counts < 1.5 lakhs/cumm. Three processes can cause thrombocytopenia, namely: Deficient platelet production, accelerated platelet destruction, and abnormal pooling of the platelets within the body. Of these, accelerated platelet destruction is the most common cause for thrombocytopenia and has variety of etiologies. The usefulness of bone marrow analysis in assessing accelerated platelet destruction is still debated. Therefore, a new simple and non-invasive diagnostic approach for thrombocytopenia is needed. Aims and Objectives: The present study was done with an aim to evaluate the use of platelet indices, namely, mean platelet volume (MPV), Platelet Distribution Width (PDW), and Platelet Large Cell Ratio (P-LCR) in differentiating the various causes of hyperdestructive thrombocytopenia. Materials and Methods: This was a prospective study conducted over a period of 2 years and consisted of 206 cases of hyperdestructive thrombocytopenia. After recording relevant clinical details, platelet count along with platelet indices – MPV, PDW, and P-LCR was recorded. Based on the etiopathology identified, cases were categorized into three groups: Group I: Immunologic – cases of Immune thrombocytopenic purpura (ITP), Group II: Non-immune: Cases of sepsis and other non-immune causes of platelet destruction, and Group III: Viral and parasitic infections. Platelet indices were compared between the study groups and the control group which included 100 healthy individuals. Comparison was done among the three study groups as well. Results & Conclusions: Dengue accounted for the highest number of 131 (89.72%) cases in the study. MPV, PDW, and P-LCR were significantly higher (P < 0.0001) when compared to the healthy controls except P-LCR in Group II.
