Disease outbreaks caused by Vibrio parahaemolyticus in Puerto Montt, Chile, began in 2004 and reached a peak in 2005 at 3,600 clinical cases. Until 2006, every analyzed case was caused by the serovar O3:K6 pandemic strain. In the summer of 2007, only 475 cases were reported; 73% corresponded to the pandemic strain. This decrease was associated with a change in serotype of many pandemic isolates to O3:K59 and the emergence of new clinical strains. One of these strains, associated with 11% of the cases, was genotypically different from the pandemic strain but contained genes that were identical to those found on its pathogenicity island. These fi ndings suggest that pathogenicity-related genes were laterally transferred from the pandemic strain to one of the different V. parahaemolyticus groups comprising the diverse and shifting bacterial population in shellfi sh in this region.

In Chile Vibrio parahaemolyticus has been detected in 3 gastroenteritis outbreaks since 1998. The most recent outbreak occurred during the summer of 2005, affecting over 10.000 people of whom one died. Affected individuals presented with one or more of the following symptoms: diarrhea, nausea, vomiting, abdominal pain and/or fever. Fecal white blood cells were detected in only 6% of patients. The predominant serotype in the 3 outbreaks was the pandemic O3:K6 strain. Diagnosis was confirmed by isolation and identification of V. parahaemolyticus in stool cultures and/or by establishing an epidemiological link. V. parahaemolyticus isolates were 100% susceptible to tetracycline, ciprofloxacin and chloramphenicol, and universally resistant to ampicillin. Due to the public health impact of the 2005 outbreak, the Ministry of Health called for a National Task Force mandated to review epidemiological, clinical and microbiological features of the outbreak and to propose management guidelines.Key words: Vibrio; Vibrio parahaemolyticus; Foodborne disease; Seafood; Diarrhea; Diarrhea outbreak.Palabras claves: Vibrio; Vibrio parahaemolyticus; Enfermedad transmitida por alimentos; Mariscos; Diarrea; Brote epidémico de diarrea.El género Vibrio, de la familia Vibrionaceae, posee más de 48 especies, y su taxonomía está en constante revisión gracias a la incorporación de técnicas de biología molecular. The work was aimed to study the microbial quality of the seafood sold in the domestic markets and incidence of Vibrio parahaemolyticus. Samples comprising of shellfish, finfish, and cephalopods were collected from various fish markets in and around Cochin. Presumed V. parahaemolyticus were identified by standard biochemical tests, and further confirmed by polymerase chain reaction targeting species-specific tl gene (450 bp). About 81% of the samples were found to exceed the limits specified for total plate count while total presumptive V. parahaemolyticus count was above the limit in 71% of the samples ranging from 5.5 9 10 5 to 9.7 9 10 7 and 0.31 9 10 2 to 7.8 9 10 6 cfu/g, respectively. Pathogenicity of the identified isolates was confirmed by Kanagawa phenomenon and urease activity. A total of 10% of the isolates exhibited weak haemolysis on Wagatsuma agar, and 1% of the isolates showed urease activity using Christensen's urea agar. Random amplified polymorphic DNA analysis revealed two major clusters based on the species rather than seasonality. The gel pattern revealed 8-10 bands ranging from 0.45 to 3.0 kb. Antibiogram results revealed 85% of the strains sensitive to chloramphenicol and nitrofurantoin. Multiple antibiotic resistance index was found to be 0.4 thus suggesting the risk potential involved in consuming seafoods. The present study has clearly demonstrated the need to adopt seafood safety measures for the products meant for human consumption.
