Characteristics of Adenovirus Pneumonia in Korean Military Personnel, 2012–2016

Abstract: Adenovirus (AdV) can cause severe pneumonia in non-immunocompromised host, but limited data exist on the distinctive characteristics of AdV pneumonia in non-immunocompromised patients. We evaluated distinctive clinico-laboratory and radiological characteristics and outcomes of AdV pneumonia (n = 179), compared with non-AdV pneumonia (n = 188) in Korean military personnel between 2012 and 2016. AdV pneumonia patients had a higher rate of consolidation with ground-glass opacity (101/152) in lobar distribution (8… Show more

“…Co-infection with bacteria was present in 27% of the patients, which is similar to the numbers reported in other studies [10]. In two cases, the concomitant bacterial findings were regarded as significant and likely to contribute to the patients' symptoms.…”

“…Another limitation is that no AdV typing was performed at the time of sampling, and samples were not available for retrospective analyses. Other studies have shown that AdV-55 is common in severe infections in healthy patients [4,6,10,12]. A prospective study would be needed to estimate the true incidence of AdV pneumonia in healthy and immunocompromised adults, and to establish if specific serotypes are over-represented in immunocompetent individuals.…”

“…A study by the US military showed that 10% of recruits at boot camp were infected with Adv, and 90% of patients with pneumonia were infected with adenovirus [10,11]. In 2006, a study by the South Korean military reported that the prevalence of Adv was 61% among military recruits with acute respiratory symptoms [12][13][14][15][16]. Upper respiratory tract infections caused by Adv may also progress to pneumonia.…”

“…Upper respiratory tract infections caused by Adv may also progress to pneumonia. Recently, several case series were reported describing recruits that died in boot camp due to severe adenoviral pneumonia in South Korea since 2012 [13,[15][16][17]. Even if mortality results or clinical outcome of Adv infection could be affected by selection bias, it has been reported that Adv infection may be severe with a higher incidence of progression to respiratory and multiorgan failure compared to other viral etiologies with the exception of influenza.…”

“…Adenovirus is a double-stranded DNA virus widely distributed in nature. It is mainly transmitted through the respiratory tract, causing not only upper respiratory tract infections but also pneumonia [1] . Adenovirus pneumonia is more common in children under 5 year old and immunocompromised people [2,3] and may progresses rapidly to severe pneumonia, acute respiratory distress syndrome, and even death.…”
