Objective: To assess changes in hyoid, tongue, pharyngeal airway, and head posture in patients who had mandibular setback surgery by intraoral vertical ramus osteotomy (IVRO) and to investigate the influence of LeFort I osteotomy. Materials and Methods: Sixty patients with skeletal Class III malocclusion were evaluated. All patients had mandibular setback surgery via IVRO, and 45 patients had additional maxillary impaction surgery via LeFort I osteotomy. Lateral cephalograms were taken before, immediately after, approximately 1 month after, and at least 1 year after surgery. Parameters indicating the hyoid, tongue, pharyngeal airway, and head posture were evaluated. Results: The hyoid significantly moved inferoposteriorly immediately after surgery and relapsed superoanteriorly during observation periods. The tongue significantly moved posteriorly during all periods. The final position of the hyoid and tongue was significantly posterior, and the final pharyngeal airway was significantly narrower compared with its presurgical position. Significant cervical hyperflection occurred during observation periods and was strongly correlated with anterior movement of the hyoid. The hyoid and tongue showed similar positions regardless of the presence of different genders or LeFort I osteotomy after the long-term observation period. Conclusions: The hyoid and tongue moved posteriorly after mandibular setback surgery via IVRO, and there was a tendency to relapse back to its original position. However, the final pharyngeal airway width remained narrower after the long-term observation period. Based on our results, careful monitoring of the airway may be needed after mandibular setback surgery via IVRO. (Angle Orthod. 2010;80:302-308.)

Resumo Introdução:A configuração e as dimensões da via aérea superior são determinadas pelas estruturas anatômicas como: tecidos moles, musculatura e esqueleto craniofacial, que compõem ou circundam a faringe. As alterações anatômicas dos tecidos moles e/ou do esqueleto craniofacial poderão tornar a via aérea superior (VAS) mais estreita. A cirurgia ortognática, que é utilizada na correção das deformidades dento-esqueléticas, leva também a alterações na VAS. Objetivo: Em vista dos fatos apresentados, o presente artigo tem como objetivo realizar uma revisão da literatura sobre as alterações da via aérea superior em pacientes classe III submetidos à cirurgia ortognática. Metodologia: Foram selecionados artigos originais e de revisão publicados entre 1990 e 2010, em duas bases de dados bibliográficos -Literatura Internacional em Ciências da Saúde (Pubmed) e Porta de Periódicos CAPES. Resultados: Foram selecionados trinta e nove (39) artigos para a escrita dessa revisão. Conclusão: A via aérea superior e a deformidade dento-esquelética classe III devem ser avaliadas cuidadosamente antes da cirurgia ortognática e sempre que o planejamento cirúrgico permitir deve-se preferir os avanços maxilares aos recuos mandibulares. Palavras INTRODUÇÃOO tratamento ortodôntico-cirúrgico é cada vez mais utilizado no tratamento de pacientes portadores de deformidade dento-esquelética. Quando essa modalidade de tratamento é adotada, a estética e função devem ser cuidadosamente avaliadas. A deformidade dento--esquelética classe III é resultado de um prognatismo mandibular e/ou uma deficiência maxilar. Historicamente, a correção cirúrgica dessa deformidade dava-se apenas pela cirurgia de recuo mandibular. Sabe-se que o excesso ântero-posterior da mandíbula isoladamente ocorre somente em 20-25% dos casos. Atualmente as cirurgias bimaxilares ou de avanço maxilar isolado têm sido utilizadas com mais frequência para o tratamento de pacientes com deformidades classe III. Na última década as cirurgias de recuo mandibular têm diminuído e passaram a representar somente 10% dos procedimentos, enquanto que as cirurgias bimaxilares e de avanço maxilar são realizadas no restante dos pacientes 1,2,3,4 . (J Korean Assoc Oral Maxillofac Surg 2012;38:9-13) Introduction: To evaluate the 3-dimensional changes in the pharyngeal airway of skeletal class III patients after bimaxillary surgery. Materials and Methods: The study sample consisted of 18 Korean patients that had undergone maxillary setback or posterosuperior movement and mandibular bilateral sagittal split osteotomy setback surgery due to skeletal class III malocclusion (8 males, 10 females; mean age of 28.7). Cone beam computed tomography was taken 1 month before and 6 months after orthognathic surgery. Preoperative and postoperative volumes of the nasopharyngeal, oropharyngeal, and laryngopharyngeal airways and minimum axial areas of the oropharyngeal and laryngopharyngeal spaces were measured. Moreover, the pharyngeal airway volume of the patient group that had received genioplasty advancement was compared with the other group that had not. Results: The nasopharyngeal and laryngopharyngeal spaces did not show significant differences before or after surgery. However, the oropharyngeal space volume and total volume of pharyngeal airway decreased significantly (P<0.05). The minimum axial area of the oropharynx also decreased significantly. Conclusion:The results indicate that bimaxillary surgery decreased the volume and the minimum axial area of the oropharyngeal space. Few studies on threedimensional analysis using computed tomo grapy (CT) reported that the pharyngeal airway space had insignificant change or even had increased after mandibular setback surgery 24. Such disagreement was believed to have resulted from the difference in the corres ponding movement direction of the maxilla.The main purpose this retrospective study is to evaluate the changes of pharyngeal airway volume and minimal axial area in skeletal class III patients undergoing mandibular setback and maxillary setback or posterosuperior movement surgery, which is expected to decrease the pharyngeal airway space. Additionally, the effect of genioplasty advancement on pharyngeal airway was evaluated.
