Changes in Clinical Characteristics of Community-Acquired Acute Pyelonephritis and Antimicrobial Resistance of Uropathogenic Escherichia coli in South Korea in the Past Decade

Abstract: This study examined changes in the clinical characteristics of community-acquired acute pyelonephritis (CA-APN) in South Korea between the period 2010–2011 and 2017–2018. We recruited all CA-APN patients aged ≥19 years who visited eight hospitals in South Korea from September 2017 to August 2018, prospectively. Data collected were compared with those from the previous study in 2010–2012, with the same design and participation from 11 hospitals. A total of 617 patients were enrolled and compared to 818 patients… Show more

“…We showed that carbapenems and aminoglycosides, such as amikacin, seem to be good choices for the treatment of serious infectious diseases of ESBL-E, though they may introduce other complicating factors such as the need to closely monitor renal response. Previous studies have shown that the proportion of carbapenem-resistant producing-Enterobacteriaceae in UTIs is less than 3% [ 26 28 ]. However, in this study, we found that carbapenem-resistant producing-Enterobacteriaceae could be as high as 5%, especially in the ESLB-E group, the resistance rate of carbapenems was 16.2%.…”

“…Interestingly, a recent study on antibiotic resistance in E. coli isolated from community onset acute pyelonephritis patients in Korea—in 2017 and 2018—showed that resistance rates against FQ and cefotaxime were 33.5% and 34.8%, respectively 30 . The fecal carriage rate of ARGs in this study represents the resistance rates of E. coli in community infections.…”

“…Data on APN are scarce: most publications provide global results from all isolates without specific reference to APN. In a recent study from Korea, ESBL-producing E. coli was isolated in up to 29% of cases of community-acquired APN [ 22 , 23 ]. In Spain, ESBL-producing strains of E. coli and Klebsiella pneumoniae have caused an increase in the prevalence of multidrug-resistant isolates of these bacteria in recent years, both in hospitals [ 10 ] and in the community [ 24 ].…”
