High-sensitivity cardiac troponin T (hs-cTnT) and the Synergy between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX) score are emerging as important prognostic markers in stable coronary artery disease (CAD). We aimed to investigate the relationship between hs-cTnT and extent and complexity of CAD assessed with STNTAX score. Measurements were obtained from 411 patients with stable CAD (Mean age = 61.7 ± 9.9 years, male/female = 247/164). The patients were divided into two groups according to the median hs-TnT value (hs-cTnT low group < 9.65 pg/ml and hs-cTnT high group ≥ 9.65 pg/ml). SYNTAX score values were higher in hs-cTnT high group compared with hs-cTnT low group (p < 0.05). Hs-cTnT was independently associated with SYNTAX score (β = 0.661, p<0.001) and diabetes (β = 0.107, p = 0.031) in multiple linear regression analysis. The cutoff value of hs-cTnT obtained by the ROC curve analysis was 9.62 ng/L for the prediction of higher SYNTAX score. In multivariate logistic regression analysis, hs-cTnT, age, diabetes, creatinine and hs-CRP were an independent predictor for higher SYNTAX score. Despite very low circulating concentrations, changes in hs-cTnT concentrations are associated with extent and complexity of CAD and presence of diabetes in patients with stable CAD.

Background: Detectable levels of troponins are often found in serum of patients with atrial fibrillation (AF), and recent reports suggest that Tn concentrations are independently related to patient prognosis. Hypothesis: We hypothesized that treatment with common rate-reducing drugs might lower the levels of cardiac troponin T (TnT) in patients with permanent AF. We also wanted to investigate whether the different drugs would impact the Tn levels differently. Methods: Sixty patients were included (mean age 71 ± 9 years, 18 women) in this randomized crossover study. All patients had stable, permanent AF without ischemic heart disease or congestive heart failure. Diltiazem 360 mg, verapamil 240 mg, metoprolol 100 mg, and carvedilol 25 mg were administered once daily for 3 weeks, in a randomized sequence. At baseline and on the last day of each treatment period, TnT concentrations were measured at rest and after a maximal exercise test. Results: TnT was detectable in all patients. In 22% of the patients, TnT concentrations were above the threshold normally used for diagnosing myocardial infarction. All drugs reduced the levels of TnT significantly compared with baseline (P < 0.001 for all), but there were no significant differences between the treatments. Levels of TnT increased significantly in response to exercise testing (P < 0.001 for all).Conclusions: Elevated TnT was demonstrated in a large proportion of stable patients with permanent AF without ischemic heart disease. A moderate reduction of heart rate by the study drugs was associated with a significant reduction in levels of TnT.

PurposeThe blood concentration of cardiac troponin T (ie, high-sensitivity cardiac troponin T [hs-cTnT]), measured using a highly sensitive assay, represents a useful biomarker for evaluating the pathogenesis of heart failure or predicting cardiovascular events. However, little is known about the clinical significance of hs-cTnT in metabolic syndrome. The aim of this study was to examine the factors affecting hs-cTnT elevation in Japanese metabolic syndrome patients.Patients and methodsWe enrolled 258 metabolic syndrome patients who were middle-aged males without a history of cardiovascular events. We examined relationships between hs-cTnT and various clinical parameters, including diagnostic parameters of metabolic syndrome.ResultsThere were no significant correlations between hs-cTnT and diagnostic parameters of metabolic syndrome. However, hs-cTnT was significantly correlated with age (P<0.01), blood concentrations of brain natriuretic peptide (P<0.01), reactive oxygen metabolites (markers of oxidative stress, P<0.001), and the cardio–ankle vascular index (marker of arterial function, P<0.01). Furthermore, multiple regression analysis revealed that these factors were independent variables for hs-cTnT as a subordinate factor.ConclusionThe findings of this study indicate that in vivo oxidative stress and abnormality of arterial function are closely associated with an increase in hs-cTnT concentrations in Japanese metabolic syndrome patients.
