Background/aim: Diabetic foot ulcers and related complications are a major cause of morbidity and hospital admissions. Our aim was to evaluate the risk factors associated with poor outcome in diabetic foot ulcers. Materials and methods:A prospective study was conducted on patients with diabetic foot ulceration attending the Madinah Teaching Hospital from June 2014 to December 2015. Potential risk factors and laboratory test results at presentation were recorded and their association with outcome (healing vs. amputation) was analyzed using IBM SPSS Statistics for Windows, Version 22.0.Results: In total, 112 patients were studied during our study period. The majority of the patients were male (60.7%) and aged 50 years and older (62.5%). Regarding the outcome, 68% healed completely, 27.7% underwent amputation, and 4.5% died during this period. Patient age of 50 and older, long duration of diabetes (>10 years), rural origin, and heel ulcers were significantly associated with poor outcome (P < 0.05). Conclusion:Patients with diabetes should have a detailed annual foot examination; those having risk factors for poor outcome require more frequent foot care, patient education, and early referral to tertiary care centers.

During the recent decades, soaring progresses in vascular disease knowledge, particularly in critical limb ischemia (CLI) treatment, enhanced novel diagnostic and interventional strategies with high serviceableness in patient's selection, arterial recanalization, and dedicated ischemic ulcer follow-up. However, despite undeniable advances in medical technology and clinical judgment, limb salvage, the ambulation recovery, and patient's survival seem only scarcely affected in this heterogeneous CLI group, particularly concerning the diabetic and renal patients. Innovative strategies such as "end artery occlusive disease" treatment or "wound-targeted revascularization" were equally proposed by following the angiosomal anatomical distribution associating individual foot collateral assessment in a unified macro-and micro-circulatory judgment. However, despite encouraging clinical results, prospective evidence still lacks on this concern. It also appears that specific wounds could not always stand for the lowest perfusion areas according to current CLI criteria, since severe neuropathy, inflammatory swelling, local infection, and skin trauma may add complementary hindrances to tissue viability.The present chapter endeavor to summarize main available treatment principles for ischemic ulcer recovery that every modern practitioner eventually disposes in an updated contemporary view."

Diabetic Foot Ulcer (DFU) is considered as one of the main causes of hospitalization in diabetic patients, which can lead to infection, gangrene, amputation, and even death if the necessary care is not provided. Based on the recent investigations, one of the most effective tactics to manage diabetic foot ulcer is to form a multidisciplinary team including a general practitioner, a nurse, an educator, podiatrists, and some consultants. Although all team members play their own important roles in reduction of DFU and amputation incidence, the role of nurses seems to be more essential. In the developed countries, the diabetes nursing is divided into several categories which one of them is diabetic foot specialist nurse. Totally, these specialists play an effective role in prevention, care, and rehabilitation of the patients with diabetic foot ulcer. Today, the specialist nurses provide effective foot ulcer management and treatment in many countries; however, these specialists, in some developing countries such as Iran, highlight a lack of specialist training as an obstacle to effective healthcare provision. Therefore, the current study aimed to emphasize on the important role of specialist nurses in the improvement of patients with diabetic foot ulcer to encourage the authorities in order to take appropriate strategies.
