The pilosebaceous unit of the human skin consists of the hair follicle and the sebaceous gland. Within this "mini-organ", the sebaceous gland has been neglected by the researchers of the field for several decades. Actually, it was labeled as a reminiscence of human development ("a living fossil with a past but no future"), and was thought to solely act as a producer of sebum, a lipid-enriched oily substance which protects our skin (and hence the body) against various insults. However, due to emerging research activities of the past two decades, it has now become evident that the sebaceous gland is not only a "passive" cutaneous "relic" to establish the physico-chemical barrier function of the skin against constant environmental challenges, but it rather functions as an "active" neuro-immuno-endocrine cutaneous organ. This review summarizes recent findings of sebaceous gland research by mainly focusing on newly discovered physiological functions, novel regulatory mechanisms, key events in the pathology of the gland, and future directions in both experimental and clinical dermatology.

ÖZKemoterapi ilaçlarına bağlı akneiform döküntünün en iyi bilinen nedeni epidermal büyüme faktör reseptör (EBFR) inhibitörleridir. Bu ilaç grubunda başlıca monoklonal antikorlar olan setuksimab ve panitumumab, tirozin kinaz inhibitörleri olan gefitinib ve erlotinib yer alır. EBFR inhibitörlerine bağlı dermatolojik yan etkiler içinde akneiform döküntü en sık ve en erken görülen reaksiyondur. Setuksimaba bağlı başlıca yan etkiler; akneiform erüpsiyon, asteni, karın ağrısı ve bulantı-kusmadır. Yazıda metastatik kolon kanseri tedavisi için setuksimab tedavisi başlanan hastada, tedavinin sekizinci günü ani gelişen yüz, boyun ve göğüste akneiform lezyonları olan 59 yaşında erkek hastayı sunuyoruz. Setuksimaba bağlı akneiform erüpsiyon tedavisinde oral tetrasiklin, topikal ketokonazol ve lokal klindamisin fosfat ve benzoil peroksit karışımı ile başarılı bir tedavinin mümkün olduğunu paylaşmak için bu olguyu sunmayı uygun gördük. ABSTRACTThe most common known causes of acneiform lesions secondary to chemotherapy are epidermal growth factor receptor (EGFR) inhibitors. Cetuximab and panitumumab which are monoclonal antibodies and gefitinib and erlotinib which are tyrosine kinase inhibitors are the major drugs in this group. An acneiform lesion is the most common and the earliest reaction among the dermatological side effects related to EGFR inhibitors. Acneiform eruption, asthenia, stomach ache and nausea-vomiting are side effects related to cetuximab. We reported a 59-year-old man who presented with sudden acneiform lesions on his face, neck and chest at the 8th day of cetuximab treatment which was used for metastatic colon cancer. We reported this case to share that it is possible to treat acneiform eruption related to cetuximab with oral tetracycline, topical ketoconazole, local clindamycin phosphate and benzoyl peroxide.

Introduction. Folliculitis is the most common side effect of epidermal growth factor receptor (EGFR) inhibitors (EGFRIs). It is often apparent, altering patients' quality of life and possibly impacting compliance. Variations in terms of the treatment‐related incidence and intensity have not been fully elucidated. Tetracyclines have been recommended for the prophylaxis and treatment of folliculitis but their efficacy is yet to be established. Materials and Methods. We carried out two systematic literature reviews. The first assessed the preventive and curative efficacy of tetracyclines. The second assessed the incidence of grade 3–4 folliculitis in the main clinical studies published. Results. In four randomized studies, preventive tetracycline treatment was associated with a significantly lower incidence of grade 2–3 folliculitis and a better quality of life in three of the four studies. In curative terms, tetracycline efficacy was not evaluated in any randomized study, but an improvement in grade ≥2 folliculitis was reported in case series. The frequency and severity of folliculitis seem to be greater with the antibodies than with the tyrosine kinase inhibitors. Analysis restricted to lung cancer studies showed a statistically greater incidence in terms of grade 3–4 folliculitis with cetuximab (9%) and erlotinib (8%) than with gefitinib (2%) (p < .0001). Conclusion. Unless contraindicated, a tetracycline should be routinely prescribed prophylactically for patients treated with an EGFRI (level of evidence, B2). In curative therapy, the level of evidence for tetracycline efficacy is low (level of evidence, D). The incidence of grade 3–4 folliculitis induced by EGFRIs appears to be lower with gefitinib.
