Cervicographs in the Management of Labour in Primigravidae

Abstract: Summary The application of an Action Line drawn four hours to the right of the Alert Line on the cervicograph of Rhodesian African primigravidae, with an active form of management for those whose cervicograph crossed the Action Line, has efficiently differentiated patients requiring Caesarean section for cephalopelvic disproportion or incorrectable inefficient uterine action from those who can be safely delivered vaginally. The regime outlined in this paper has reduced the Caesarean section rate, the incidence… Show more

“…These results are similar to those found in a previous study (4) , which showed high rates of vaginal deliveries (89.7%) in zone I and of cesarean sections in zone III (20.6%). Another study, performed in China (18) , obtained higher rates (50.0%) for cesarean sections when compared to the present study results for deliveries that crossed the action line.…”

“…A study performed in Central Africa (3)(4) added two lines to the partogram; namely alert line and action line, which determined the limits to specific partogram zones: zone I, to the left of the alert line; zone II, between the alert and action lines; and zone III, when the action line is crossed. The referred lines permit to identify any abnormal labor evolution and distinguish eutocic labor from dystocic delivery.…”

“…The action line is parallel, and 4 hours to the right of the alert line. Several studies [14][15][16] , but not all 17 , have shown that the partogram reduces the risk of prolonged labor, cesarean sections and perinatal mortality in developing countries.…”

“…A straight alert line (cervix dilatation of 1 cm/hour) and a straight action line (shifted 4 hours parallel to the right) were added to the cervicograph, representing cervical dilatation ( Figure 5) 51,52 .…”
