Cervical spine fractures and rear car seat restraints.

Abstract: Examination in hospital showed bruising with the imprint of the harness buckle under his chin, torticollis, and painful limitation of movement. No abnormal neurological signs were present in the limbs but he had slight difficulty with micturition for several weeks after an initial period of urinary retention lasting 24 hours.Radiographs of the cervical spine showed a fracture at the junction of both pedicles and lateral masses of C2 (Hangman's fracture). “…Hangman's fracture and congenital spondylolysis of the cervical axis in children are rare entities, with only several reported cases in peer reviewed literature (n ϭ 24; 11 cases of spondylolysis and 13 cases of a hangman's fracture, mean age approximately 2 years and 6 months, range 2 months to 16 years) (1)(2)(3)(4)(5)(6)(7)(8)10,(12)(13)(14)(15)(16)(17)(18)(19)(20). As discussed in previous publications, the differentiation between these two diagnoses can be very difficult (4,9).…”

“…Agran (1987) indicated that about 21% of lap-shoulder belted children (aged lo-14 years) in motor vehicle crashes sustained cervical strain. Restrained children with cervical spine injuries and no head impact have been reported (Conroy and Hall 1987;Diekema and Allen 1988;Fuchs, et al 1989). A child's body dimensions and his/her proportions are markedly different from those of an adult, and therefore, the child must not be considered a scaled-down adult (Burdi et al 1969).…”
