Cerebral haemangioblastoma

A case of supratentorial haemangioblastoma is presented. Diagnostic examinations, including computerized axial tomography, were not accurate. Histological examination of the removed tissue disclosed the correct diagnosis. Supratentorial haemangioblastomas are very rare tumours. We describe a case with regard to preoperative neuroradiological investigations, computerized axial tomography, and final histological diagnosis.

SYNOPSIS A retrospective study has been carried out on 67 patients harbouring 78 posterior fossa haemangioblastomata. The site, macroscopic nature, and histological features are described, and correlations sought with the degree of erythrocytosis in the peripheral blood. The problems of aetiology are discussed and a single hypothesis advanced.Haemangioblastomata are histologically benign tumours of the neuraxis. There are two aspects which are remarkable-the association with the von Hippel-Lindau complex and the association with erythrocytosis of the peripheral blood. Although the latter is well documented no attempt has previously been made to relate the histological features to the haematological findings. In order to do so, and to attempt a single hypothesis regarding aetiology, a retrospective study of posterior fossa haemangioblastomata has been carried out. The details of clinical presentation, the von Hippel-Lindau complex and surgery have been reported previously (Jeffreys, 1975). METHODSSixty-seven patients harbouring 78 posterior fossa haemangioblastomata were studied. Seventy-four tumours were histologically verified and the sections were re-examined. Three of the unverified tumours were deemed irremovable at operation though they were associated with other removable and histologically verified posterior fossa tumours. The last tumour occurred in a patient with erythrocytosis and in whom a cyst only was found at operation. Light and ultrastructural features of a cerebellar hemangioblastoma in a 56-year-old man are described in detail. The neoplasm was composed of three major cell types: endothelial cells, pericytes, and stromal cells. The endothelial cells lined the fenestrated vascular channels. The pericytes were ensheathed by their own basal lamina which separated them from the basal lamina covering the endothelium. The stromal cells contained, in addition to the conventional organelles, numerous membrane-bound lipid inclusions, annulate lamellae, and nuclear bodies. There were also present transitional cells which shared the fine structure of all the three major cell types. Histogenetically, the tumor was considered to be of vascular origin. The stromal cells represented the stem cells which, under the neoplastic influence, continued to proliferate and differentiate into "vasoformative" elements (pericytes and endothelium) which formed new blood vessels. The transitional forms between the stromal cells and the "vasoformative" elements suggested that the cellular components of a hemangioblastoma shared a common ancestry, most likely of an angioblastic lineage.
