Mutations in the von Hippel-Lindau (VHL) gene are responsible for VHL disease, congenital polycythemia, and are found in many sporadic tumor types as well. Reports of VHL mutations are dispersed throughout original articles and databases that have not been recently updated. We compiled a comprehensive mutation table of 1,548 germline and somatic VHL mutations, derived from this protein of only 213 amino acids. We describe detailed phenotype and gene mutation information for 945 VHL families, including 30 previously unpublished kindreds from The Netherlands (six novel mutations). These data represent the most extensive catalog of germline VHL mutations to date. We also review VHL disease, known and theorized pathogenesis of common VHL manifestations, and genotype-phenotype correlations. Analysis of all VHL families, excluding germline mutations resulting in congenital polycythemias, describes the spectrum of mutation types: 52% missense, 13% frameshift, 11% nonsense, 6% in-frame deletions/insertions, 11% large/complete deletions, and 7% splice mutations. Esta seção é parte de um projeto de educação continuada em Endocrinologia e Metabologia e está sendo publicada graças ao patrocínio e a colaboração daO OBJETIVO DESSA APRESENTAÇÃO é a discussão do caso de uma paciente com a Síndrome de von Hippel-Lindau, sob os seus diferentes aspectos clínicos, laboratoriais, radiológicos e terapêuticos. A seguir será apresentado o estudo genético realizado na paciente e serão discutidas a principais alterações gênicas envolvidas no aparecimento dessa sín-drome clínica.Unitermos: Von Hippel-Lindau; Feocromocitoma; Tumor de pâncreas; Angioma de retina; Hemangioblastoma SNC; Tumor de rim APRESENTAÇÃO DO CASOPaciente 33 anos, feminina, branca com quadro de cefaléia holocraniana, intensa e constante há 3 anos. A investigação detectou tumor de cerebelo que foi operado e o exame anátomo-patológico revelou se tratar de hemangioblastoma de cerebelo. Com esse diagnóstico, a paciente foi encaminhada para o nosso serviço para investigação de síndrome de von Hippel-Lindau. Negava hipertensão arterial e paroxismos adrenérgicos sugestivos de feocromocitoma. Negava diabetes, distúrbios gastrointestinais, alterações visuais, auditivas e neurológicas. Tabagista de 3 cigarros por dia e etilista social. Negava uso regular de qualquer medicação. Antecedentes familiares: irmã de 31 anos, operada de feocromocitoma unilateral aos 18 anos, com quadro atual de hipertensão arterial; pai diabético, falecido aos 42 anos após quadro súbito de vômitos seguido de coma (provável acidente vascular cerebral); tios paternos cardiopatas, um deles teve morte súbita aos 40 anos durante jogo de futebol e o outro, vivo, teve infarto do miocárdio; mãe com 63 anos é hipertensa leve, controlada com Higroton ® .Exame físico: bom estado geral, corada, hidratada, eupnéica, acianóti-ca, anictérica, afebril, peso=59kg, altura=l,60m, IMC=23, P (supina)=72bat/min, PA (supina)=120/90mmHg, P (ortostática)=80bat/min, PA (ortostática)=130/90mmHg. Auscultas cardíaca e pulmonar normais. Tireóide não palpável. Ausência de adenomegalias. Abdome sem massas ou visceromegalias. Membros sem edema com pulsos presentes e simétricos. Pele sem alterações. Ausência de déficits motores ou sensitivos. Fundo de olho normal. Mapeamento de 24 horas da pressão arterial revelou ausência do descenso noturno e pressão diurna no limite superior da normalidade.Avaliação Laboratorial: Determinações em urina de 24horas: VMA=4,4 e 10,2mg/24h (N=2 a 12 mg/24h); Metanefrinas=0,61 e 0,14 mg/mg creatinina (0,05 a 1,2 mg/mgcr); Noradrenalina=233 e 333mg/24h (14 a 80pg/24h); Adrenalina=14mg/24h (0,5 a 20mg/24h); Dopamina=213 e
