Celsior Versus University of Wisconsin Preserving Solutions for Liver Transplantation: Postreperfusion Syndrome and Outcome of a 5‐Year Prospective Randomized Controlled Study

Abstract: While retaining the same degree of safety and effectiveness as UW for LT, CS may yield postliver graft reperfusion benefits, as shown in this study by a significant reduction in the incidence of PRS and greater metabolic control.

“…The findings indicate that Celsior has comparable efficacy and safety to University of Wisconsin solution for liver graft preservation within a median ischemia time of 7 h. These results verified an earlier pilot, randomized prospective trial suggesting that University of Wisconsin and Celsior have comparable efficacy [56]. A recent 5-year prospective RCT demonstrated similar safety and effectiveness between University of Wisconsin (n ¼ 51) and Celsior solution (n ¼ 51) [17]. A recent 5-year prospective RCT demonstrated similar safety and effectiveness between University of Wisconsin (n ¼ 51) and Celsior solution (n ¼ 51) [17].…”

“…Also, a prospective, nonrandomized, multicenter, European registry study suggested Celsior and University of Wisconsin solutions provided similar liver allograft survival [57]. Importantly, this study also detected a reduction in postreperfusion syndrome in the livers stored with Celsior versus University of Wisconsin solution [17]. “…The current accepted interpretation of this phenomenon is the 'second strike theory'. A large number of alcohol intake induces the formation of alcoholic fatty liver as a 'first strike', and the alcoholic liver injury requires a second strike, including the change in nutrition, the effect of drug poisons, viral infections and genetic factors (19). The result showed that Quercetin significantly reduced AST, ALT, TBIL and TG content in ALD mice.…”

“…Compared with Celsior preservation solution, UW is associated with less ischemic necrosis in early cardiac allograft biopsies [89] and improved short-term outcomes [90]. UW was superior to histidine-tryptophan-ketoglutarate (HTK) solution after prolonged cold preservation of renal grafts [91] but inferior to Celsior in liver transplantation associated with more post-reperfusion syndrome [92]. Adenosinelidocaine preservation solution supplemented with melatonin and insulin improved cardiac output, heart rate, and blood pressure following cold storage of rat hearts for 8 h. Adenosine-lidocaine alone was more efficacious than Custodial HTK and Celsior preservation solutions [93].…”
