Cellulitis due to Streptococcus pneumoniae with Diminished Susceptibility to Penicillin in an Immunocompromised Patient

Abstract: A 74-year-old man with multiple myeloma developed facial and cervical cellulitis and severe sepsis as a complication of surgery (alar region basal cell carcinoma). The etiological agent was, surprisingly, penicillin-resistant Streptococcus pneumoniae (PRSP). The patient successfully received 16 days of antibiotics. Amoxicillin was given as monotherapy during the last 14 days of treatment. PRSP can be responsible not only for otitis media, pneumonia or meningitis, but also for various other types of infection i… Show more

“…L'hyperleucocytose à polynucléaires neutrophiles est classique. La diminution de la CRP serait plus précoce en cas d'évolution favorable (7). La flore anaérobie à l'état physiologique est saprophyte de la cavité buccale et de l'oropharynx (8).…”

“…S. Pneumoniae cellulitis is usually treated with Penicillin. The emergence of drug-resistant pathogen has made the treatment more challenging especially in patients who are immunocompromised [12].…”

“…Diagnosis requires the demonstration of the pathogen in culture either from purulent material of cellulitis positive in 86% of cases or from blood cultures positive in 96% in the review by Parada and Maslow, highlighting the importance of obtaining blood cultures 2 . Treatment of pneumococcal cellulitis relies on penicillin, but cellulitis was the result of a penicillin‐resistant strain in two cases 5,15 . Surgical treatment was required in 50% of patients because of suppurative complications with debridement, fasciectomy, amputation and skin graft 2,3 .…”
