Cell-Based Therapy for the Treatment of Glioblastoma: An Update from Preclinical to Clinical Studies

Abstract: Glioblastoma (GB), an aggressive primary tumor of the central nervous system, represents about 60% of all adult primary brain tumors. It is notorious for its extremely low (~5%) 5-year survival rate which signals the unsatisfactory results of the standard protocol for GB therapy. This issue has become, over time, the impetus for the discipline of bringing novel therapeutics to the surface and challenging them so they can be improved. The cell-based approach in treating GB found its way to clinical trials thank… Show more

“…Human T cells are lymphocytes that belong to the adaptive immune system. So far, majority of the GBM CAR T therapies have been autologous treatments with single or multiple dose (3)(4)(5)(6) infusions. This approach features the delivery routes of CAR T cells which are intracranial at the tumor site (ICT), intracranial into the ventricles (ICV) and intravascular (IV) administration (34).…”

“…In addition to NSCs, another type of adult stem cell, the mesenchymal stem cell, is being widely tested in the treatment of GBM. MSCs are multipotent stem cells that can be obtained from bone marrow, peripheral blood, adipose tissue, umbilical cord, cord blood, dental pulp, et al (6). They are spindle-shaped cells which can be relatively easily isolated, expanded extensively during in vitro culture while maintaining their potential to generate different types of cells (141).…”

“…On the other hand, BBB is composed of endothelial cells with tight junctions and adhesion molecules that forms a barrier between blood and brain extracellular fluid. BBB serves as an obstacle to many small and large molecules, including drugs and other therapeutic agents ( Patel and Patel, 2017 ; Attia et al, 2021 ). To better cross BBB, either BBB must get more permeable, or the approach to drug delivery must be modified ( Hsu et al, 2021 ).…”

“…Current therapy only minimally improves the median survival time of patients, from 12 months to 14.6 months, while the 5-year survival rate of treated patients is less than 10% [ 24 ]. As a result, there is an imperative need for the development of new therapies that are targeted and effectively effective in the treatment of glioblastoma [ 25 ]. Typically, the challenges raised by glioblastoma are found in the molecular and genetic signaling pathways happening in this type of cancer [ 26 ].…”
