CD34 expression in primary cutaneous malignant melanoma: apropos of a case and review of the aberrant melanoma phenotype

Abstract: We present a case of malignant melanoma of nodular subtype, which strongly expressed CD34. The spectrum of abnormal phenotypes in malignant melanoma is reviewed, and a possible explanation for the presence of CD34 is discussed. This case demonstrates the potential of malignant melanoma to express CD34, defining an infrequently recognized aberrant phenotype. Whether or not expression of this marker is associated with a more aggressive clinical course remains to be determined.

“…Before 2007, only two examples of CD34-positive cutaneous melanomas could be found in the literature. 136,137 Reactivity for CD34 in melanomas of other locations had also rarely been reported. 138 Recently, a series of cutaneous melanomas showing CD34 expression in one third of the 30 cases of invasive melanoma studied has been published.…”

“…Other commonly used vascular markers are factor VIII‐related antigen and CD34. Factor VIII‐related antigen is negative in melanoma; however, though infrequently, melanoma can have strong reactivity for CD34 . Further, it must be kept in mind that CD31, CD34, and/or factor VIII‐related antigen are lost in up to 40% of angiosarcomas, further demonstrating the need for a panel of immunostains.…”

“…CD34 is a marker commonly seen on hematopoietic cancer stem cells (35) and has more recently been shown be present on tumor-initiating, self-renewing melanoma cells in mouse models (10). Additionally, pathologic analysis of malignant human melanoma biopsies has shown that CD34 is expressed (36,37). ABCB5 is a drug efflux transporter which regulates skin progenitor cell fusion and is a well-known marker of melanoma stem cells.…”
