The cysteine proteinases, μ-and m-calpain, along with their inhibitor, calpastatin, have been hypothesized to play a role in skeletal muscle protein degradation. Because nutrition has previously been shown to influence the expression of calpastatin, the working hypothesis of this study was that the quantity and source of dietary protein could influence regulation of the calpain system in muscle. The objectives to support this hypothesis were to determine the effects of dietary protein (amount and source) on the expression of calpastatin in canine skeletal muscle. This study comprised eight diets with seven dogs per diet. A biopsy was taken from the biceps femoris of all 56 dogs before and after 10 wk on their respective diets. This experimental design allowed examination of change within individual dogs. Diets 1 to 4 contained 12% total protein derived from chicken and/or corn gluten meal in ratios of 100:0, 67:33, 33:67, and 0:100%, respectively. Diets 5 to 8 contained 28% total protein with protein sources and ratios identical to Diets 1 to 4. Differences in calpastatin were examined qualitatively using SDS-PAGE and immunoblotting, and semiquantitatively with densitometric analyses. The majority of the calpastatin blots showed three distinct calpastatin bands, the uppermost appearing at approximately 110 kDa. Diet 5 (28% CP, 100% chicken) resulted in an increase in the expression of the 110-kDa calpastatin band compared with the other two lower molecular weight bands in the same samples. Muscle from dogs fed Diet 5 showed greater increase in (P < 0.05) calpastatin intensity of the topmost band than those fed Diet 8 (0:100; chicken:corn gluten meal). Diet 5 (100:0; chicken:corn gluten meal) showed greater total calpastatin intensity than Diet 8 (0:100; chicken: corn gluten meal). These data suggest that dogs fed a diet containing a higher total percentage of chicken protein may have a greater potential to regulate calpain-mediated degradation of muscle protein than dogs fed diets containing corn gluten meal. ResumoO tumor Walker-256 é um carcinoma de crescimento rápido e tem sido estudado sob vários aspectos metabólicos, associados ou não, à caquexia. Foi observado, em nosso laboratório, que em animais portadores de tumor Walker-256 após morte espontânea (geralmente em torno do décimo quinto dia) apresentavam edema pulmonar significativo com presença de líquido na cavidade pleural. Alguns trabalhos têm sugerido que o treinamento físico melhora a sobrevida de animais com tumor e minimiza os efeitos da caquexia. O objetivo de nosso trabalho foi o de avaliar o índice de edema pulmonar e massa muscular esquelética e cardíaca, além da sobrevida de ratos portadores de tumor Walker-256 submetidos previamente a treinamento físico por natação (N). Para este estudo, foram usados ratos Wistar machos (200 a 220g), submetidos ao treinamento físico por natação (1 hora; 5 dias/semana, 4 semanas). Um dia após o treinamento, ratos sedentários (C) ou treinados (N) foram submetidos à inoculação no flanco direito de 8 x 10 7 células de tumor de Walker 256 (T). Imediatamente após a morte espontânea desses animais, foram avaliados o índice de edema pulmonar (IEP), a massa muscular esquelética (gastrocnêmio e soleus) e cardíaca. O edema pulmonar foi avaliado pelo índice calculado pela relação entre os pesos pulmonar e corporal de cada animal, e multiplicada por 100 (PP/PC x 100) (LEE et al., 2001). O índice de massa muscular (IMM) foi calculado de forma similar. Em animais normais, o IEP é igual a 0,53±0,02 (n=20). Em ratos portadores de tumor após a morte espontânea apresentaram IEP significativamente maior (2,62±0,31, n=18). Após o treinamento físico em animais sem tumor, o IEP foi de 0,55±0,03 (n=5). Já em animais portadores de tumor previamente treinados obteve-se um índice de edema inferior ao grupo controle com tumor (1,46±0,16, n=5; p<0,05). Em relação à massa muscular, verificou-se, como esperado, uma diminuição no índice de massa muscular do gastrocnêmio no grupo controle com tumor, quando comparado ao grupo sem tumor (C=0,53± 0,01 ; CT=0,38± 0,02). O mesmo ocorreu para o músculo soléus (C=0,11±0,01; CT=0,08± 0,01). O treinamento físico não modificou o IMM de todos os músculos avaliados quando comparados ao controle sem tumor. No entanto, no rato treinado com tumor, o IMM do músculo gastrocnêmio foi maior quando comparado ao 1 Profa. Dra. Terapia nutricional enteral em politraumatizados sob ventilação mecânica e oferta energética

OBJETIVO: O objetivo deste estudo foi avaliar a adequação energética dos pacientes politraumatizados em suporte ventilatório internados na unidade de terapia intensiva de um hospital público de Porto Alegre (RS), por meio da comparação entre as calorias prescritas e as efetivamente administradas, assim como entre as calorias estimadas pela equação de Harris-Benedict e a prescrição energética de cada paciente. MÉTODOS: Estudo de coorte prospectivo de pacientes politraumatizados, simultaneamente sob ventilação mecânica e terapia nutricional enteral. Verificou-se o tempo de permanência sob ventilação mecânica e a oferta energética durante o período de terapia nutricional enteral. A associação entre as variáveis quantitativas foi avaliada através do teste de correlação de Spearman devido à assimetria das variáveis. RESULTADOS: Foram acompanhados 60 pacientes, na faixa etária de 18 a 78 anos, sendo 81,7% do sexo masculino. Os tempos medianos de internação hospitalar, permanência na unidade de terapia intensiva e ventilação mecânica foram de 29, 14 e 6 dias, respectivamente. A média do percentual de dieta administrada foi de 68,6% (DP=18,3%). Da amostra total, 16 (26,7%) pacientes receberam no mínimo 80% de suas necessidades diárias. Não houve associação estatisticamente significativa entre o valor energético total administrado e os tempos de ventilação mecânica (r s=0,130; p=0,321), de unidade de terapia intensiva (r s=-0,117; p=0,372) e de internação hospitalar (r s=-0,152; p=0,246). CONCLUSÃO: Os pacientes incluídos neste estudo não receberam com precisão o aporte energético prescrito, ficando expostos aos riscos da desnutrição e seus desfechos clínicos desfavoráveis.
