Case 257: Leiomyosarcoma of the Inferior Vena Cava

Abstract: History A 65-year-old woman presented to her primary care physician with a history of progressive abdominal pain mainly in the upper quadrants, nausea, and edema in the bilateral symmetric lower extremities. Other symptoms and use of medication or related drugs were denied. Physical examination findings were normal. The serum lactate dehydrogenase level was 302 U/L (5.04 μkat/L) (reference range, <247 U/L [4.12 μkat/L]), and all other laboratory data were within normal ranges. Electrocardiography and chest rad… Show more

“…Moreover, contrasted‐enhanced CT and MRI show heterogeneous enhancement within the mass, which is due to necrosis or hemorrhage. Extraluminal leiomyosarcoma can invade surrounding tissues and should be differentiated from retroperitoneal tumors or peripheral organ tumors . A few articles reported the sonographic features of IVC leiomyosarcoma as a predominantly solid and lobulated retroperitoneal mass, sometimes containing small cystic components .…”

“…Primary inferior vena cava (IVC) leiomyosarcoma is a rare soft tissue malignancy originating from the smooth muscle media, with poor prognosis. Prevalence is three to five times greater in women than in men . Less than 400 cases have been reported so far.…”

“…In intraluminal type of LMS, IVC lumen is dilated and filled with mass [10]. In extra luminal type of LMS, the primary tumour of the IVC grows exophytic, which is often misdiagnosed as tumour of the adjacent organs like adrenal gland mass or sarcoma arising from retroperitoneum with direct invasion into IVC [11].…”

“…Nevertheless, many case reports have used MRI for IVC LMS diagnosis. 8,[12][13][14][15][16][17][18] Compared with CT, MRI can provide more accurate information about the tumour's location, extent, collateral vessels, and adjacent structures with superior soft-tissue resolution, special sequences, and safer contrast material. The T2-weighted black blood sequence and high concentration contrast agent can overcome the limitations of CT, clearly revealing the vessel walls and lumens of IVC and collateral vessels.…”
