Carotid Plaque Characteristics and Disease Activity in Rheumatoid Arthritis

Abstract: Patients with RA had more CP compared with controls and patients in CDAI remission, and controls had more stable CP than patients with active disease; these findings point to the importance of achieving remission in RA.

“…98 Plaque vulnerability is associ ated with rheumatic disease activity, demonstrating a link between inflammation and plaque composition. 98,99 Plaques in patients with RA who have active disease are more vulnerable than in those with RA in clinical remission. 99 Compared with matched controls without RA, patients with RA have worse outcomes after acute coronary syndrome, with increased deaths and increased and earlier recurrence of MI.…”

“…In patients with RA, the severity of carotid atherosclerosis is associated with disease activity. 26,27 A recent study highlighted the association among RA flare, cumulative burden of RA disease severity, and increased CVD risk. 56 RA, together with other inflammatory arthritis, may be good models to study the role of inflammation in promoting atherosclerosis.…”

“…Accordingly, novel markers of CVD such as impaired insulin sensitivity, endothelial dysfunction, carotid intima-media thickening, and aortic stiffness, have all been associated with inflammation and are found to occur with higher prevalence in patients with CIDs [39][40][41][42][43][44][45][46]. Indeed, the inflammatory mechanisms implicated in the pathogenesis of CIDs resemble the chronic inflammatory processes that also drive the development of atherosclerosis [13,14].…”
