Cardioversion of acute atrial fibrillation in the short observation unit: comparison of a protocol focused on electrical cardioversion with simple antiarrhythmic treatment

Abstract: Electrical cardioversion of recent-onset AF in the SOU is safe, effective and reduces hospitalisations. Further studies are needed to identify the most cost-effective strategy for the management of AF patients in emergency settings.

“…Effectiveness of ECV in our registry was comparable to that reported in the literature (78-97%) [7][8][9][10]. Although some data suggest that in AF, 200 J energy should be started, especially in non--paroxysmal AF [11], we show that about 160 J energy may also be effective.…”

“…AF pathways have been tested in mostly academic settings in the United States and Canada and have met primary safety endpoints. These pathways include strategies for early rhythm control, initiation of rate control regimens, and anticoagulation for stroke prophylaxis initiated in the ED . Many of these services can be delivered in the ED or in an observation unit, decreasing the use of inpatient hospital services.…”
