IntroductionFew data are available about pediatric cardiovascular disease (CVD) in the Horn of Africa. The objective of this study was to describe the spectrum of CVD in children in Djibouti.MethodsClinical features and management of Djiboutian children between 1 month-old and 15 year-old with CVD were prospectively recorded over a two-year period in Bouffard Military Hospital in Djibouti (January 2009- December 2010).ResultsClinical examination and echocardiography were performed on 156 patients: 32 of them (20%) had CVD. Three (10%) of them had Down's syndrome. The median age was 5 years (male 53%). Congenital heart disease was observed in 27 (84%) patients and dilated cardiomyopathy (DCM) in 5 (16%) patients including 2 patients with rheumatic valvular disease. Ventricular septal defect was frequent (28%). Other abnormalities were atrial septal defect (13%), Tetralogy of Fallot (9%), pulmonary stenosis (6%) and 3 other patients had multiple congenital anomalies condition. Surgical management was required in 22 (69%) patients and was performed on 15 (47%) cases. During follow up (mean 11.3 ± 6.8 months), 5 (16%) patients died. Absence of surgery was associated with significant mortality (p > 0.05) but age, sex and mean follow up were not.ConclusionPediatric CVD is at least as common in this Djiboutian community as in other African cohorts. The absence of surgery was a major mortality risk factor. DCM was frequent in this study. Much work remains to be done to discover the size and nature of genetic and environmental contributions to these various forms of heart diseases in the Horn of Africa.

Advances in cardiac surgery techniques and early diagnosis have enabled the increased survival of individuals with congenital heart disease. The investigation of the quality of life in children and adolescents with congenital heart disease provides complementary information to clinical data that can assist in decision making on the part of health professionals. Although many studies have been conducted to investigate the quality of life of children and adolescents with congenital heart disease, the results prove to be contradictory; while some studies show that congenital heart disease can impact the quality of life, others describe a better perception of quality of life among children and adolescents who suffer from the disease when compared with healthy control subjects. The purpose of this study is to review the literature on the assessment of health related quality of life in children and adolescents with congenital heart disease, in order to systematize the existing knowledge on this topic today. It is observed that research seeks to investigate aspects of personality in cardiac patients, their coping strategies used and perceived social support, aiming at better understanding the association of these variables with the level of quality of life in this population.

ResumoAs cardiopatias representam importantes causas de internação e óbitos em crianças e adolescentes e o conhecimento de informações de âmbito epidemiológico são determinantes para tomada de decisão nos serviços de saúde. O objetivo do estudo foi investigar as características epidemiológicas e clínico-hospitalares de crianças e adolescentes internadas em um hospital infantil na cidade de Manaus-Amazonas. Estudo transversal, de base hospitalar e abordagem quantitativa, com obtenção de dados a partir de 173 prontuários de crianças de 0 a 19 anos com diagnóstico médico de cardiopatia no período entre 2011 a 2015, com as variáveis socioeconômicas, demográficas e clínico-epidemiológicas, utilizando-se análise estatística descritiva de frequência absoluta e relativa. Os resultados apontam predominância das cardiopatias do sexo masculino 60,6%, raça parda 31,7% e idade menor de um ano 68,2%. Tipo de cardiopatia com maior frequência foi a acianótica 86,1%, tendo como principal motivo de internação o comprometimento do sistema respiratório 43,8%. Em 70,5% dos casos houve necessidade de transferência para a Unidade de Terapia Intensiva, sendo 48,6% por complicações relacionadas ao sistema cardiovascular. A maioria das mães desempregadas, ensino fundamental incompleto, renda inferior a um salário mínimo e obtendo de quatro ou mais filhos. Conclui-se que há um índice elevado de cardiopatias congênitas entre a faixa etária estudada, principalmente em menores de cinco anos, pertencentes a famílias com fatores socioeconômico desfavoráveis, com complicações na maioria dos casos, demandando serviços de alta complexidade, sugerindo a existência de uma política pública de saúde incipiente no município, de difícil acesso aos exames diagnósticos e tratamento cirúrgico.Palavras-chave: Cardiopatia Congênita. AbstractCardiopathies represents significant cause of hospitalization and death in children and teenagers, and the knowledge of epidemiologic information is determinant for making a decision in health service. The main idea of the study was to feature the epidemiologic and clinical characteristics of children and teenagers from a children's hospital in the city of Manaus -Amazonas. A cross-sectional clinical study with a quantitative approach, gathered data from 173 medical records of children from 0 to 19 years old diagnosed with heart disease between 2011 and 2015 with demographic, clinical epidemiologic, and socio-economic variables, using descriptive statistics of relative and absolute frequency. The results revealed that cardiopathies were predominantly found in brown (31.7%) males (60.6%) younger than 1-year old (68.2%). The main kind of cardiopathy was acyanotic (86.1%), being the reason for 43.8% of hospitalization due to respiratory impairment. In 70.5% of the cases, it was necessary moving to the Intensive Care Unit, and 48.6% of which were due to complications related to the cardiovascular system.
