The causes of pulmonary arterial hypertension (PAH) are diverse and include multiple congenital and acquired cardiac diseases as well as diseases primarily affecting the pulmonary vasculature, lung, pleura and chest wall. The traditional role of CT in evaluating PAH includes assessment of pulmonary vasculature and lung parenchyma with limited assessment of the heart. Advances in multidetector CT technology with improved spatial and temporal resolution now permit accurate delineation of cardiac morphology. CT pulmonary angiography (CTPA) is widely utilised in the workup of patients with suspected pulmonary vascular disease and can identify both pulmonary and cardiac causes. As the initial presentation for CTPA is often precipitated by nonspecific, unexplained symptoms and therefore undertaken by a general radiologist, it is important that a systematic approach to the interpretation of these studies, including cardiac evaluation, is routinely adopted. This paper reviews the CT evaluation in pulmonary hypertension with a particular focus on the cardiac causes, their subclassification into congenital systemic to pulmonary shunts and secondary to left heart disease, and their imaging features. It emphasises the use of a systematic approach to interpretation of CTPA examinations both in patients with known PAH and those with previously unsuspected disease.

To identify multi-detector computed tomographic (MDCT) features discriminating bicuspid aortic valves (BAVs) from tricuspid aortic valves (TAVs) in patients with aortic valvular disease using surgical findings as reference. Forty-five patients underwent ECG-gated cardiac MDCT scans prior to aortic valve replacement. Morphologic patterns of aortic valves on MDCT were classified into: bicuspid without raphe (A), fused valve with a fish-mouth opening (B), fused valve without a fish-mouth opening (C), and tricuspid without fusion (D). To differentiate congenital raphe of BAV from commissural fusion of TAV, MDCT features of patterns B and C were evaluated. Diameters of the aortic root and ascending aorta between patients with BAVs and TAVs were also compared. Patterns A (n = 6) and B (n = 6) were all bicuspid, in pattern C: 8 of 26 (30.8%) were bicuspid, and pattern D (n = 7) were all tricuspid. In patterns B and C, uneven cusp size, round-shaped opening and midline calcification at leaflet fusion were strongly associated with BAVs (all, P < 0.05). The mean length of leaflet fusion in BAVs was significantly larger than in TAVs (13.5 vs. 8.7 mm, P < 0.0001), with a cutoff value of 10.3 mm providing a sensitivity of 85.7%, a specificity of 83.3%, and an area under the ROC curve of 0.90. In all patients, the mean diameter of the ascending aorta was larger in patients with BAVs than with TAVs (43.3 vs. 39.7 mm, P < 0.05). MDCT features of uneven cusp size, round-shaped opening, midline calcification, longer leaflet fusion and larger diameter of the ascending aorta can be helpful in distinguishing BAVs from TAVs.

PreambulumA Magyar Kardiológusok Társasága Kardiovaszkuláris Képalkotó Munkacsoportjának és a Magyar Radiológusok Tár-sasága Szív Képalkotó Diagnosztikai Szekciójának konszenzusdokumentumának 2013-as megjelenése óta a koroná-ria komputertomográfi ás angiográfi a (CCTA) jelentős fejlődésen ment keresztül. A technikai újítások eredményeként funkcionális vizsgálatok végzése vált lehetővé, továbbá nemzetközi vizsgálatok eredményei alapján a szakmai ajánlá-sok a CCTA új indikációval bővültek. Mindezen változások miatt a Magyar Kardiológusok Társasága Kardiovaszkuláris Képalkotó Munkacsoportja szüksé-gét érzi, hogy a felvételek értelmezéséhez, valamint a lelet elkészítéséhez korábban megfogalmazott szakmai állásfog-lalását a Society of Cardiovascular Computed Tomography (SCCT) által kiadott ajánlás alapján átdolgozza (1). Jelen dokumentumnak nem célja a CCTA indikációinak és kontraindikációinak részletezése. Tekintettel a kardiovaszkuláris betegségek nagyszámú megjelenési formájára és az egyes esetek közötti magas variabilitásra, az alábbiakban részletezett irányelvektől, ajánlásoktól egyedi mérlegelés alapján eltérhetünk. BevezetésA koszorúér-betegség kimutatására alkalmazott noninvazív képalkotó módszerek közül az egyik legmodernebb eljárás a CCTA-vizsgálat, amelynek segítségével direkt információt nyerhetünk a koronáriákat érintő ateroszklerotikus plakkokról és a lumenszűkület mértékéről. A megfelelően kivitelezett, jó minőségű CCTA igen nagy szenzitivitással (kb. 95-100%) és specifi citással (>90%) képes azonosítani a súlyos fokú koszorúér-szűkületeket. A vizsgálat negatív prediktív értéke átlagosan 95-100% (2, 3).Az ateroszklerotikus folyamatok okozta koronáriaszű-kület mértékének meghatározásában az invazív koronária-angiográfi át (ICA) tekintjük referencia standardnak.
