Cardiac 123I-Metaiodobenzylguanidine (MIBG) Scintigraphy in Parkinson’s Disease: A Comprehensive Review

Jamir Pitton Rissardo,
Ana Letícia Fornari Caprara

Abstract: Cardiac sympathetic denervation, as documented on 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy, is relatively sensitive and specific for distinguishing Parkinson’s disease (PD) from other neurodegenerative causes of parkinsonism. The present study aims to comprehensively review the literature regarding the use of cardiac MIBG in PD. MIBG is an analog to norepinephrine. They share the same uptake, storage, and release mechanisms. An abnormal result in the cardiac MIBG uptake in individuals with p… Show more

“…In drug-induced parkinsonism, a reduction in dopamine transporter binding may be observed, and this reduction is reversible upon discontinuation of the offending medication. Neuroimaging findings should be interpreted in conjunction with the clinical presentation and history of medication use [199]. A clear temporal relationship between the initiation of a specific drug and the onset of parkinsonian symptoms can strengthen the suspicion of drug-induced parkinsonism.…”

“…SPECT imaging is advantageous in assessing changes in brain perfusion in conditions such as Huntington's disease and other neurodegenerative disorders. Cardiac 123Imetaiodobenzylguanidine (MIBG) scintigraphy has already proven helpful in supporting the diagnosis of Parkinson's disease [118].…”
