Cowper's glands are pea sized glands present inferior to the prostate gland in the male reproductive system. They produce thick clear mucus prior to ejaculation that drains into the spongy urethra. Though it is well established that the function of the Cowper's gland secretions is to neutralize traces of acidic urine in the urethra, knowledge regarding the various lesions and associated complications of this gland is scarce. This review provides a comprehensive report on the development, function and various lesions associated with Cowper's gland.

Resumen: Descripción del caso clínico: Varón de 47 años con sintomatología inespecífica del tracto urinario inferior. Se le practica una uretroscopia en la que se aprecia un pequeño orificio en la porción inferior de la uretra bulbar por el que no se puede progresar. En la uretrografía retrograda se observa una pequeña cavidad de 8 mm bajo la uretra, compatible con siringocele. Se completa estudio mediante uretrosonografía retrograda donde se aprecia una cavidad de 22x4 mm (APxL) sin apreciar el orificio. Finalmente, mediante uretrosonografía retrograda con contraste ecográfico se aprecio el orificio y un tamaño del siringocele de 35x5 mm (APxL). Relevancia: El caso pone de manifiesto como las nuevas técnicas de imagen pueden contribuir en el estudio de la patología uretral. Implicaciones clínicas: El hecho de que el siringocele sea muy poco frecuente en adultos y en muchas ocasiones asintomático puede provocar que su estudio mediante los protocolos habituales no sea optimo produciéndose falsos negativos, infraestimaciones de su tamaño, errores en su clasificación o errores diagnósticos. Conclusiones: La uretrosonografía retrograda simple y la uretrosonografía retrograda con contraste ecográfico son técnicas de utilidad en el estudio del siringocele.

Metastatic Cancer of Cowper's Gland: A Rare Cancer Managed Successfully by Molecular Profiling

Cancer of Cowper's gland is a very rare cancer. This case represents the 9th case in the medical literature. As such, there are no phase II or phase III trials to guide treatment. In this article, we report the successful treatment of a patient over a 7-year period guided solely by molecular profiling. Through multiple cycles to treatment, the cancer was controlled using drugs targeting c-kit, as the cancer steadily increased the expression of c-kit. This report also documents the use of a novel drug combination based on sunitinib that was well tolerated and may warrant testing in other c-kit-dependent cancers.
