To evaluate changing trends in patient collectives, age-related patterns of manifestation, and diagnostic pathways of patients with extrapulmonary head and neck tuberculosis (TB), and to provide strategies to fasten diagnosis in these patients.Study design: Case control study. Methods: A 10-year retrospective analysis of 35 patients diagnosed with extrapulmonary TB in the head and neck at a tertiary university institution from 2009 to 2019, with special focus on the influence of the patient's age on consideration of TB and clinical patterns.Results: The vast majority of patients younger than 40 years had their origin in countries with high TB burden (P = .0003), and TB was considered very early as a differential diagnosis (P = .0068), while most patients older than 40 years were domestic citizens initially suspected for a malignancy, who more often had an underlying immunosuppressive condition (0.0472). Most frequent manifestations in both groups were the lymph nodes, larynx, and oropharynx. Surprisingly, no differences in the rates of open TB or history of TB infection in the family anamnesis were found. Conclusion:The two groups of patients found most often are younger patients migrating from regions with high TB burden and elderly domestic patients suffering from immunosuppressive conditions, with the latter often being misdiagnosed as malignancies. TB remains an important but difficult differential diagnosis, due to the initially unspecific symptoms and the great variety in the presentation of manifestations in the head and neck.

Purpose: To identify factors associated with lymphoma in patients with anterior Mycobacterium tuberculosis (TB). Methods: A retrospective case-control analysis was performed in a highly tuberculosis-endemic area. TB-infected patients were retrospectively identified when a history of TB exists before the diagnosis of lymphoma. Inpatients with lymphoma (n=1,057) and benign tumors (n=12,916) were consecutively enrolled at Xinjiang Medical University Cancer Hospital between January 2016 and December 2019. Results: The proportion of TB in patients with lymphoma (n=148, 14.0%) was significantly higher than that in the control (benign tumor) group (n=175, 1.4%) (p<0.0001). The frequency of TB infection in patients with Hodgkin’s lymphoma, B-cell non-Hodgkin’s lymphoma (NHL), and T/NK-cell NHL is 13.6%, 14.6%, and 11.9%, respectively. Compared to other subtypes of B-cell NHL, a relatively high proportion of tuberculosis was found in patients with chronic lymphocytic leukemia /small lymphocytic lymphoma, marginal zone B-cell lymphoma, diffuse large B-cell lymphoma, corresponding to 20.6%, 18.6% and 15.3%, respectively. For T-cell NHL, the proportion of TB infection in patients with peripheral T-cell lymphoma and anaplastic lymphoma kinase-positive T-cell NHL was 26.1% and 20%, respectively. Multivariate analysis revealed male gender as an adverse risk factor was associated with lymphoma after tubercular infection. In addition, male gender and old age (>60 years) was associated with the B-cell NHL. Conclusion: A high proportion of Mycobacterium tuberculosis infection was found in patients with lymphoma. In TB-infected patients, older age and male gender are associated with susceptibility to lymphoma, suggesting the awareness of a screening program might be useful for early detection of lymphoma.

Objective: We systematically review the molecular mechanism of the interaction between lung cancer (LC) and tuberculosis (TB), and put forward the existing problems in order to provide suggestions for early intervention and future research direction.Background: TB and LC are two global public health problems affecting human health. LC is the main cause of cancer-related death worldwide and TB is one of the leading causes of death among infectious diseases, especially in resource-poor areas. Previous studies have suggested that a history of TB may be associated with an increased risk of LC. With the improvement of LC treatment, the occurrence of pulmonary tuberculosis in the course of LC treatment is also frequently reported recently. Methods:The molecular immunological mechanisms of interaction between LC and TB, and related epidemiological literature are reviewed. The research progress and problems to be solved are summarized.Conclusions: Chronic inflammation, immune abnormalities, scar formation, gene mutations and drug effects caused by TB may be associated with the occurrence of LC induced by abnormalities in various molecular pathways. LC and decreased immunity during treatment may also increase the risk of latent TB activation or new TB infection through immune pathways. Data on dual burden areas of TB and LC are still lacking, and more clinical studies are needed to elucidate the association.
