Cancer-Attributable Mortality Among People With Treated Human Immunodeficiency Virus Infection in North America

Abstract: Approximately 10% of deaths in PWHIV prescribed ART during 1995-2009 were attributable to cancer, but this fraction increased over time. A large proportion of cancer-attributable deaths were associated with non-Hodgkin lymphoma, lung cancer, and liver cancer. Deaths due to NADCs will likely grow in importance as AIDS mortality declines and PWHIV age.

“…Another study, performed by the North American AIDS Cohort Collaboration on Research and Design (NA-ACCORD) on more than 46,000 subjects receiving antiretrovirals from 1995 to 2009, estimated that 9.8% of deaths were due to cancer. The fraction of deaths due to NADCs was 7.1%, while only 2.6% were attributable to AIDS-defining cancers [9]. More recent studies showed higher proportional mortality ratios, associating more than 10% of all deaths with NADCs [10].…”

“…They also have a more severe clinical course of cancer and lower survival rate compared to the noninfected population . In HIV+ patients, 10–20% of all deaths are attributable to cancer …”

“…2,3 In HIV+ patients, 10-20% of all deaths are attributable to cancer. 4,5 Given the higher risks for HIV-positive population, developing cancer control strategies for this group is a rising challenge to public health. To provide the context for further research, we will discuss clinical aspects related to the cancer burden in patients with HIV-infection and highlight details on the role of antiretroviral drugs in the development of cancer, which is not limited to viral suppression.…”

“…Hepatocellular carcinoma (HCC) is a growing cause of cancer death among people living with HIV infection. 1 Driven largely by hepatitis C virus (HCV) coinfection, hepatitis B virus (HBV) coinfection, and alcoholic liver disease, the incidence of HCC among HIV-infected persons in North America has risen more than 4-fold from 1995-2009. 2 Moreover, HIV-infected individuals have a 4-fold higher risk of HCC than uninfected persons.…”
