Can Some Anticancer Treatments Preserve the Ovarian Reserve?

Abstract: Background Preventing premature ovarian failure (POF) is a major challenge in oncology. With conventional regimens, cytotoxicity‐associated POF involves primordial follicles (PF) pool depletion by apoptosis or overactivation mechanisms, notably mediated by the ABL/TAp63 and PI3K/Akt/mTOR pathways. New anticancer treatments have been designed to target pathways implicated in tumor growth. Although concerns regarding fertility arise with these targeted therapies, we hypothesized that targeted therapies may exert… Show more

“…The division, proliferation, and apoptosis of GC coexist under normal circumstances, but follicular atresia occurs when GC apoptosis >10% [ 30 ]. At the same time, once the process of follicular atresia becomes out of control, it would accelerate apoptosis, which causes premature PF depletion [ 31 ]. Autophagy induces apoptosis by triggering the accumulation of autophagosomes in the GC and luteal cells, also leading to follicular atresia [ 32 ].…”

“…The question may be asked at the time of the pre-transplant consultation and some practitioners may realize that a sperm freezing was not performed at the time of the diagnosis of the hematological disease. A specialized opinion has to be taken to discuss the solution on a case-by-case basis, depending on the previous treatments received and the planned conditioning [ 42 , 43 ]. A remote post-transplant spermogram can be performed.…”

“…Further study is also being done on the dose-independent and dose-dependent natures of infertility secondary to lower abdominal radiation therapy and total body irradiation [26]. Vallet et al [27] worked to review current evidence of antineoplastic regimens causing premature ovarian failure, especially with regards to newer targeted therapies, which may not necessarily deplete the primordial follicles. These targeted therapies may include tyrosine kinase inhibitors (Imatinib, Sunitinib), mTor inhibitors (rapamycin), AS101, proteasome inhibitor (Bortezomib), and glycogen synthase kinase 3 inhibitors, which are typically employed as co-treatment alongside appropriate regimens for the target disease.…”
